Abstract

ABSTRAKPenanganan kondisi disfungsi endotel sebelum terjadi kerusakan lebih lanjut sangatlah penting, karena berakibat terjadinya resistensi insulin dan hiperglikemia. Diharapkan dengan pemberian Catechins Green tea GMB-4 dapat memperbaiki kerusakan endotel melalui penghambatan oksidasi NADPH dan peningkatan produksi NO sehingga dapat mencegah terjadinya komplikasi lanjut pada pasien dengan diabetes Millitus. Desain penelitian ini true eksperimen dengan pendekatan post test only control group design. Subyek penelitian ini adalah kultur sel endotel yang berasal dari Human Umbilical Vein Endothelial Cell (HUVEC) yang dipapar glukosa 30 mM dan diberi perlakuan dengan Catechins Green tea GMB-4 dengan 3 dosis yaitu 0,03 mg/ml;0,3 mg/ml dan 3 ng/ml. Setelah pemberian selama 12 jam selanjutnya dilakukan analisa kadar NADPH dan Nitric Oxide (NO) dengan metode Colorimeter menggunakan NADPH Assay kit ab65349 dan Nitric Oxide Assay kit KGE001.Hasil penelitian menunjukan ada penurunan kadar NADPH pada kultur HUVEC setelah perlakuan Catechins Green tea GMB-4 pada dosis 0,3 mg/ml, dan terjadi peningkatan kadar Nitric oxide pada perlakuan dosis 0,03 mg/ml dan dosis 0,3 mg/ml. Hasil uji statistic one way Anova didapatkan nilai signifikan 0,000 untuk kadar NO dan NADPH. Sehingga dapat di simpulkan pemberian Catechins Green tea GMB-4 pada dosis 0,3 mg/ml dapat menurunkan oksidasi NADPH dan meningkatkan produksi NO secara signifikan. Pemberian Catechins Green tea GMB-4 dapat mengaktivasi eNOS dan berperan dalam penghambatan oksidasi NADPH melalui pospotidylinositol-3 kinase, cAMP dependent protein kinase dan jalur Akt. Catechins juga mengkatalisis produksi NO dari L-arginin dan menghambat ROS dengan jalan mengikat ion metal pada reaksi redok, menghambat transkripsi faktor, menghambat enzim prooksidan dan meningkatkan enzim antioksidan.Kata kunci: antioksidan, Catechins Green tea GMB-4, NO, NADPH ABSTRACTHandling conditions of endothelial dysfunction before further damage is very important, because it results in insulin resistance and hyperglycemia. Expected by administration of Green Tea Catechins GMB-4 can improve endothelial damage through inhibition of NADPH oxidation and NO production increased so as to prevent the occurrence of further complications in patients with diabetes millitus. True experimental design of this study to approach post-test only control group design. The subjects of this study were cultured endothelial cells derived from Human Umbilical Vein Endothelial Cell (HUVEC) were exposed to 30 mM glucose and treated with Green Tea Catechins GMB-4 with 3 doses are 0.03 mg / ml, 0.3 mg / ml and 3 ng / ml. Analized NADPH levels and Nitric Oxide (NO) with Colorimeter method using NADPH Assay kit ab65349 and Nitric Oxide Assay kit KGE001. The results showed decrease in NADPH levels in cultured HUVEC after treatment of Green Tea Catechins GMB-4 at a dose of 0.3 mg / ml, and an increase in levels of nitric oxide in the treatment dose of 0.03 mg / ml and 0.3 mg / ml . Results of one way ANOVA statistical test obtained significant value of 0.000 for the levels of NO and NADPH.. So it can be concluded giving Green tea Catechins GMB-4 at a dose of 0.3 mg / ml can reduce the oxidation of NADPH and increased NO production significantly. Provision of Green tea Catechins GMB-4 can activate eNOS and play a role in the inhibition of NADPH oxidation through pospotidylinositol-3 kinase, cAMP dependent protein kinase and Akt pathways. Catechins also catalyze the production of NO from L-arginine and inhibited ROS by binding the metal ions on the reaction redok, inhibits transcription factor, inhibits prooxidant enzymes and increases antioxidant enzymes.Keywords: antioksidant, Catechins Green tea GMB-4, NO, NADPH

Abstract

Rapid population growth in Indonesia has prompted the government to launch the Family Planning (KB) program in order to reduce the birth rate. This research was conducted to find out what factors influenced the incidence of drop out of 3 month injection contraceptive in pamekasan and analyzed the most dominant factor on the incidence of drop out Kb injection 3 months at puskesmas proppo pamekasan district (BKKBN, 2013) The research design in this research is analytical survey with cross sectional approach. Population in this research is all acceptor drop out KB injection 3 month in Pamekasan, with technique. cluster sampling obtained a sample of 149 KB injectors who dropped out in Pamekasan. Data processing with Editing, Coding and Tabulating. The analysis technique used is ordinal regression analysis using SPSS v.23 for windows.The results of this study indicate that the support of husbands, knowledge there is an effect on the incidence of drop out KB inject 3 months in pamekasan. The most dominant factor on the incidence of drop out injecting KB 3 months in pamekasan is the factor of support of husbands with the effect of 8,707 times. Keywords: support of husband, knowledge and event of drop out KB injection 3 months.

Abstract

Penyakit demam berdarah dengue (DBD)  merupakan salah satu masalah kesehatan masyarakat  di Indonesia   dan sering menimbulkan suatu  kejadian luar biasa (KLB). Salah satu yang bisa dilakukan untuk merancang program pemberantasan dan pencegahan DBD yang lebih baik adalah dengan melakukan analisis spasial dengan menggunakan Geographic Informatioon System (GIS). GIS dapat memetakan penyakit berbasis alamat penderita bermanfaat dalam melihat sebaran penyakit sehingga mampu mengidentifikasi daerah yang berisiko tinggi. Selain itu, dilakukannya analisis spasial memungkinkan suatu penyakit untuk dilihat dari berbagai konteks sehingga diharapkan mampu dilakukan perencanaan yang lebih baik dalam memberantas dan mencegah suatu penyakit. Hasil penelitian ini menunjukkan ada pengaruh faktor  keberadaan kontainer air, sanitasi lingkungan  dan kepadatan  vektor terhadap  peningkatan kasus DBD di Kota Kediri. Perilaku masyarakat berpengaruh dalam upaya pencegahan dan pemberantasan DBD terhadap terhadap kejadian penyakit  DBD  di Kota Kediri. Hasil identifikasi tipe virus DHF dengan menggunakan PCR di daerah endemis DBD kota Kediri di dapatkan serotype negative untuk Den-1, Den-2, Den-3 dan Den-4. GIS berperan dalam evaluasi program pemberantasan kasus DBD di Kota Kediri dimana dapat menggambarkan peta daerah kasus endemis DBD di kota Kediri.Kata Kunci:  Demam berdarah, Geographic Informatioon System (GIS), tipe virus dengue

Abstract

ABSTRAKHelicobacter pylori  berperan dalam infark miokard  akut dan penyakit jantung koroner.  Sitotoksin H.pylori  dapat menginduksi sitokin inflamasi IL1-β dan TNFα yang mengaktivasi enzim proteolitik MMP-9 dan menyebabkan degradasi kolagen melalui plak aterosklerosis. Penelitian ini dilakukan untuk membuktikan kemampuan whole cell H.pylori dalam menginduksi degadrasi kolagen tipe IV melalui peningkatan aktifitas makrofag. Aktivitas makrofag dilihat dari kadar TNFα dan IL1-β yang diukur dengan metode Elisa. Hasil uji ANOVA mengindikasikan perbedaan signifikan (p<0,001) aktifitas makrofag pada kelompok normal dan whole cell H.pylori. Produksi enzim MMP-9 dapat dideteksi melalui gelatine zymography dilanjutkan dengan western blotting pada berat molekul 93 kDa. Fragmenliasi kolagen didapatkan pada fragmen 61,2 kDa  dan 29 kDa. Dapat disimpulkan bahwa H.pylori dapat menginduksi degradasi kolagen tipe IV melalui peningkatan makrofag. Temuan ini juga mendukung patomekanisme infark miokard.Kata Kunci : Kolagen Tipe IV, makrofag, whole cell H.pylori

Abstract

The objective of this research was to analyze the quality of bioactivator from stale rice, cassava tape, banana bumps, cow”s rumen / rotten fruit, based on concentration and fermentation time, with factorial design. The first factor was the concentration of stale rice, tape, banana bumps, cow”s rumen or rotten fruit, which was 150 grams and 250. The second factor was fermentation time consisting of: F1 (Fermentation for 10 days), F2 (Fermentation for 20 days). Parameters observed were: total population of bacteria, pH and temperature. Data were analyzed using Anova. The results showed that the highest result of total bacterial population was found on 150 gram banana bumps with fermentation of 20 days ie 41 x 106 and the lowest on 150 gram cassava tape with 10 day fermentation that was 1x102. The highest pH was found on rice with 250 grams of fermented rice with 20 days of 3.00 and the lowest on 250 grams of banana bumps on day 0 which was 5.96. The highest temperature on stale rice was 150 gram with 10 days fermentation, 150 banana bumps and 250 gram with 10 day fermentation of 29.5 0C and the lowest temperature on 150 gram cassava tape, rumen / rotten fruit of 150 gram on day 0 was 27 0C. Abstract

Every pregnant woman would face the risk of life-threatening complications. WHO estimated that about 15% of all pregnant women would develop into complications related to their pregnancies. The purpose of this research was to explore the role of midwife in community empowerment about early detection of pregnancy complication at Proppo Public Health Center of Pamekasan Regency. This research was a qualitative research. Data collection using primary and secondary data and conducted in-depth interview. Respondent in this research was village midwife at Proppo Public Health Center as many as 17 people. The results showed that midwife's role in community empowerment about early detection of pregnancy complication at Proppo Public Health Center was varied. Village midwives at Proppo Public Health Center served as educators, counselors, facilitators are possible depending on the level of knowledge and education of the community include cadres, community leaders, pregnant women and families. Based on this research, socio-cultural role is very influential on the pattern of empowerment. So it needs to be studied more deeply about socio-cultural factors in relation to community empowerment about early detection of pregnancy complications. Abstract

Introduction: HIV still becomes health problem that threaten Indonesia and many countries around the world. When HIV enters the body, it looks for CD4 cells and begins to replicate itself. Once replicated and left the dead CD4, the new viral particles will be looking for and infecting new CD4. The purpose of this study was to determine the effects of cognitive supports on CD4 level people living with HIV. Those cognitive supports consist of information, psychological, and spiritual support in order to increase the levels of CD4.Methods: This was quasy experiment research with posttest group design. Population were all people living with HIV at Kota Kediri. Samples were 20 respondents who meet the criteria, divided into two groups, intervention and control. The independent variable was cognitive support, while dependent variable was CD4 level of people living with HIV. Blood samples were collected to examine CD4 level. Data were then analyzed by using Anova test and T-test.Results: Results indicated that the cognitive supports had an effect in increasing the levels of absolute CD4 and CD4 percentage of people living with HIV at a signifi cance value of 0.003. However, there was no signifi cant difference between the control groups taking ARV regularly and irregularly.Conclusion: The provision of cognitive support was capable to stimulate the immune system by increasing T lymphocytes that have CD4 molecular markers. Thus, an increase in the levels of CD4 is expected to improve the quality of life of people living with HIV.

Abstract

Introduction: The negative impact caused by people living with HIV/AIDS is a matter of physical, psychosocial and emotional, so that people living with HIV need to improve towards adaptive coping mechanisms. Roy explained that there are three stimulus that can influence the coping mechanisms, there are stimulus focal, contextual stimulus, stimulus residual. The purpose of this study was to determine the factors related with coping mechanisms based on the Roy Adaptation Theory.Methods: The study design using analytic correlational cross-sectional approach, 30 respondents were selected by purposive sampling technique. The research instrument used questionnaire.Results: The results showed focal stimulus, stimulus contextual, adan residual stimulus associated with coping mechanisms in people living with HIV, the majority of respondents have a negative coping mechanisms as many as 18 respondents (60.0%) and positive coping mechanisms as much as 12 respondents (40.0%).Discussion: Based on the analysis showed there is relationship stimulus focal p-value = 0.018 and contextual stimulus p-value = 0.004, while the residual stimulus doesn’t have relation with the coping mechanisms of people living with HIV in KDS friendship plus Kediri. During join the peer support groups people with HIV has a place to share the feelings and emotions and be able to communicate social well, so that their focal and contextual stimuli can stimulate a response from KDS cognator and regulators that can affect physiological functions so they can adapt adaptively.
