Abstract

Pharmaceutical installations have an important role in improving and providing effective services to achieve patient satisfaction. Patients' complaints occur if they receive a service that does not meet expectations. The hospital management must investigate the causes of patients' complaints. Solving the patients' complaints indicates that the hospital can fulfill their expectations. The purpose of this study is to examine the results of servqual on service quality, customer satisfaction index, and importance-performance analysis on service quality and complaint handling. This research employed a descriptive design with qualitative and quantitative approaches. The study was conducted by distributing questionnaires to 100 outpatients. The respondents' answers were then analyzed using the Cartesian diagram to obtain quadrants A, B, C, and D. Interviews were directly conducted with the complaint management and pharmaceutical installation staff to explore the attributes included in quadrant A (top priority). The patients were selected using the purposive sampling method. Finally, the data were analyzed using the servqual analysis, importance-performance analysis, and customer satisfaction index. The result shows that the service quality dimension has a negative gap value. The reliability dimension has the smallest gap while the responsiveness dimension has the largest gap. The result of the customer satisfaction index shows that outpatient satisfaction with the service quality and complaint handling has a value of 56.53%. Thus, this dimension is categorized as quite satisfying. The result of the importance-performance analysis shows that the dimensions included in quadrant A are the tangible evidence dimension, including poor physical facilities, and the responsiveness dimension, including patients' long waiting time to manage prescriptions and complaint handling.

Abstract

The research was to determine the drug distribution system used in pharmacy installation Jogja International Hospital and to know the factors that effect the drug distribution system in pharmacy installation Jogja International Hospital. This study used observasional method by case study design. The data was collected by retrospektif and prospektif, with interview and collection data from drug distribution document and the recipe survey in pharmacy installation Jogja International Hospital (JIH). The results showed that drug distribution system used in JIH was UDDS, individual prescription, floorstock and emergency kit. The UDDS used in JIH was not have problem.

Abstract

Hypertension is a trigger factor of chronic kidney disease. This disease requires a special incentive treatment in geriatrics patient.The complexity of therapy forcing the use of polypharmachy teraphy. Evaluation needs to be done with the aim of knowing description the typeof antihypertensive drugs used and the quantity of its use based methods Defined Daily Dose (DDD) and suitability compared with Standar Pelayanan Medis (SPM) and the JNC 7. This study was a descriptive study with retrospective data collection. The sample was medical record data regarding the use of antihypertensive drugs in hypertension with chronic renal failure in geriatric patients (age≥ 60) hospitalization. The data collected includes classes and antihypertensive drug name, strength, frequency, duration of usage, length of stay. Then the data to be processed to determine the quantity of drug use in the ambulatory DDD and Drug Utilization (DU 90 %). The results of this study concluded that antihypertensive drugs included in the DU 90 % segment in 2012 were amlodipine (39.19%), furosemide (24.14%), captopril (14.14%), telmisartan (11.36%), valsartan (2.04%), and in 2013 were amlodipine (35.11%), furosemide (22.88%), captopril (18.80%), telmisartan (7.02%), valsartan (6.51%). Suitability based on SPM in 2012 (78.57%) and in 2013 (63.63%) and by the JNC 7 in 2012 (42.85) and 2013 (36,36%). There was no change in the pattern of use of antihypertensive drugs between 2012 and 2013.

Abstract

Pandan leaves are medicinal plants that contain alkaloids, saponins, flavonoids, tanins and poliphenols compounds that have high antioxidant activity and potential as hepatoprotective. This research was conducted to determine the effect of ethanol extract pandan leaves (Pandanus amaryllifolius Roxb.) on the rate of ALP in to determine potency extract ethanol pandan leave inhibit necrosis cell of liver in white rats isoniazid and rifampisin induced. This research was used thirty rats were divided in 6 groups. Group I as a normal group. Group II as a hepatotoxic group. Group III as a drug group was given Curcuma 3,6 mg/200 g BW. Group IV, V, and VI as the treatment group given test solution ethanol extract pandan leaves 8,64 mg/200 g BW, 17,28 mg/200 g BW and 25,92 mg/200 g BW for 28 days. All groups except group I were induced by isoniazid and rifampisin 10,8 mg/200 g BW. All groups on day 0, 14th and 28th set ALP rate. Results obtained were analyzed by One Way ANOVA test. The results showed that ethanol extract pandan leaves at a dose of 8,64 mg/200 g BW, 17,18 mg/200 g BW, and 25,92 mg/200 g BW can inhibit ALP in to determine potency ethanol extract pandan leave inhibit necrosis cell of liver in white rats isoniazid and rifampisin induced. Dose of the ethanol extract pandan leaves was the most effective is 25,92 mg/200 g BW.

Abstract

Breadfruit leaves (Artocarpus altilis (Park.) Fsb.) is a medicinal plant that is thought to have potential as a hepatoprotective. In the community itself has been widely used and is reported to have many uses since it contains efficacious compounds in plants such as saponins, polyphenols, tannins, hydrocyanic acid, acetylcholine, riboflavin and flavonoids. This study aimed to determine the hepatoprotective effect of breadfruit leaves extract in mice induced by hepatitis B vaccine with the percentage of liver cell necrosis as parameter. Breadfruit leaves extract was made by percolation method with ethanol 70% as the solvent. The animal test was induced by hepatitis B vaccine, then they was grouped into 5 groups of negative control group were given CMC 1%, the positive control group were given Echinacea (Imboost®) and the treatment group of test animals were given breadfruit leaf extract at a dose 750 mg/kgBW, 1200 mg/kgBW, 1650 mg/kgBW. Hepatoprotective effect was observed by liver necrosis parameter on histopathologic liver mice preparations at day 35 after treatment. The results showed that the breadfruit leaf extract can provide hepatoprotective effect by reduction the liver necrosis. The percentage of reduction necrosis of the liver in the treatment of 70% ethanol extract of breadfruit leaves dose of 1200 mg/kgBW provides the most effective hepatoprotective effect on male mice induced hepatitis B vaccine.

Abstract

Primary health centers is a facility that organizes health efforts at the first level to achieve health degrees, but in its implementation is still constrained in realizing standard pharmaceutical services. The purpose of the research is to find out the quality of drug management in Tegal City primary Health Center. This study is a non-experimental descriptive study. The study was conducted throughout the primary health center in Tegal City. Data collection is prospective and retrospective by tracing documents to obtain secondary data and direct observations, interviews of pharmaceutical personnel, heads of primary health centers, and heads of the pharmaceutical section to obtain primary data. The next drug management indicator is done descriptive data analysis by calculating the value of the indicator with the formula then compared to the standard and inter primary health care. Indicators of drug management used 28 and those that meet standard 10. Indicator results include conformity of items with disease patterns 76.39%, planning accuracy 321.10%, the accuracy of the number of requests 169.84%, the storage of narcotics 72.92%, the storage of drugs without contamination 98.97%, the storage of high alert drugs 68.15%, the storage of LASA drugs 87.5%, ITOR 1.87 times /year, the availability of drugs 36.08 months, drug items less than 14.01%, safe drug items 37.94%, excess stock items 41.76%, non-prescribed drugs 4.59% and drug value ED 3.85%. These results show that drug management indicators in Tegal City primary health centers have not been efficient and need improvements ranging from the planning stage to control. Indicators that still need improvement include ITOR and all indicators of drug availability. Things that need to be done to improve indicators that have not been efficient include increasing the number and quality of pharmaceutical human resources in primary health centers, increasing supervision of drug use and control, building communication, and a good organizing culture in primary health centers.

Abstract

Latar Belakang: Selama kehamilan terutama pada trimester kedua dan ketiga, kolesterol dalam darah dapat meningkat 20-30%. Peningkatan kadar kolesterol pada ibu hamil dapat meningkatkan terjadinya preeklampsia. Kulit buah jeruk bali secara empiris telah digunakan dalam menurunkan kadar kolesterol. Kandungan flavonoid dalam kulit buah jeruk Bali terbukti memiliki efektivitas dalam menurunkan kadar kolesterol total dalam darah.Tujuan: dari penelitian ini adalah untuk mengetahui efektivitas kulit buah jeruk Bali sebagai penurun kadar kolesterol total untuk pencegahan preeklampsia selama kehamilan.Metode: Penelitian eksperimental dilakukan dengan penyarian sediaan teh celup kulit jeruk Bali dosis 100mg/kgBB, 200mg/kgBB dan 400mg/kgBB yang diujikan secara in vivo pada tikus putih jantan yang diinduksi diet tinggi lemak. Kadar kolesterol total darah diukur selama 21 hari dan dihitung penurunannya.Hasil: Penelitian menghasilkan dari 3 kelompok dosis teh celup kulit jeruk Bali (TCKJB) dengan dosis 100mg/KgBB, 200mg/KgBB dan 400mg/KgBB terdapat penurunan kadar kolesterol total di bandingkan kelompok hiperkolesterolemia. TCKJB 100mg/KgBB mengalami penurunan sebesar 22,4mg/dL, TCKJB 20mg/KgBB mengalami penurunan sebesar 31mg/dL, sedangkan kelompok TCKJB 400mg/KgBB mengalai penurunan sebesar 50,2mg/dL. Kata kunci: Teh celup kulit buah jeruk Bali (Citrus Maxima Merr), kolesterol total, preeklamsiaThe activity of Bali orange peel (Citrus maxima Merr) tea as a lowering of total cholesterol levels for the prevention of preeclampsia during pregnancy.Background: During pregnancy, especially in the second and third trimesters, cholesterol in the blood can increase by 20-30%. Increased cholesterol levels in pregnant women can be happen the occurrence of preeclampsia. Grapefruit rind empirically has been used in reducing cholesterol levels. The content of flavonoids in Bali orange peel is proven to have effectiveness in reducing total cholesterol in the blood.Aim: This study was conducted to determine the effectiveness of Bali orange peel as a decrease in total cholesterol levels.Method: An experimental study was carried out by extracting a dosage of Bali orange peel in doses of 100mg / kg BB, 200mg / kg BB and, 400mg / kg BB which were tested in vivo on male white rats induced by a high-fat diet. Total blood cholesterol level was measured for 21 days and a decrease was calculated.Result: The study resulted from three groups of Bali orange peel tea dosage (TCKJB) with a dose of 100mg / Kg BB, 200mg / Kg BB and 400mg / Kg BB there was a decrease in total cholesterol levels compared to the hypercholesterolemia group. Abstract

Drugs are one of the important components of the Back-Referral Program (BRP), so the guarantee of the availability of BRP drugs in pharmacies is a factor in the success of BRP implementation. The purpose of this study was to determine the consumption of antidiabetic drugs using ATC/DDD and 90% DU methods, analyze the cost of consuming antidiabetic drugs, and determine the suitability of antidiabetic drugs with the National Formulary. This study was an observational study with a cross-sectional design and retrospective data collection. The study was conducted at 6 BRP pharmacies, namely Pharmacy A, Pharmacy B, Pharmacy C, Pharmacy D, Pharmacy E, and Pharmacy F in the area of Demak Regency. The antidiabetic drug consumption data was obtained from BRP patient prescriptions from July 2020 to June 2021. The cost data were obtained based on prices in the JKN drug e-catalog system at BRP pharmacies in the area of Demak Regency. In analyzing the data, the researcher calculated DU at 90%, the cost of drugs included in the DU segment at 90%, and the suitability of drugs with the National Formulary. The results showed that the highest consumption of antidiabetic drugs in Back-Referral Program (BRP) pharmacies in the area of Demak Regency was glimepiride (54.72%) and metformin (31.01%). The highest cost of consuming antidiabetic drugs per DDD was insulin (Rp.17,639.73), the lowest cost was glimepiride (Rp.65.35), the total cost of antidiabetic drugs was Rp.53,509,090 and an incompatible drug with National Formulary was pioglitazone. In general, the highest consumption of antidiabetic drugs in Back-Referral Program (BRP) 6th pharmacies in the area of Demak Regency, which was included in the DU segment 90%, were glimepiride, metformin and the suitability of drugs with the National Formulary has not reached 100%.

Abstract

Each installation in the hospital must continue to evaluate, improvement, and develop, including the pharmacy installation. Pharmacy installations as one of the medical support services in hospitals are required to improve the quality of their services on an ongoing basis. This study aims to see, the root of the problem, and provide evaluation and improvement for the reading of critical waste in the inpatient pharmacy service process of Kabupaten Karanganyar Hospital. This study uses a non-experimental research design and was descriptive qualitative in nature. Data techniques in qualitative and quantitative research. The value of patient data was obtained from distributing questionnaires. The pharmaceutical service process was obtained from observation and document review, then mapping in the form of value stream mapping. The determination of waste was obtained from distributing questionnaires and in-depth interviews with the 5 why method. The results of the reasearch show critical waste on inpatient service in pharmacy installation is waste defect and waste waiting. The root cause of critical waste is the prescription of drugs by doctors outside the formulary and the root cause of waiting for waste is a prescription from the hospital admission at the same time. The proposed improvement for the cause of critical waste is Monitoring and evaluating the implementation of the formularium on an going basis, provide special software to enter patient data, and implement the use of electronic prescribing to shorten the service process time.
