Abstract

Background: Pain stimulates distress emergence that potentially impairs the development and growth on neonates. Heel prick brings about pain. This research aimed at observing the effectiveness of sucrose and swaddling on pain response to neonates. Methods: A clinical trial was designed by random under control using cross-over design. This study involved 24 neonates (aged 28 days) which taken from population using a consecutive sampling technique. Each subject was given two treatments of sucrose (24%) and swaddling, but the pain responses were observed at different time during and after received the heel prick (1, 2, and 3 minutes). Response of neonatal pain focused on face observation, oxygen saturation, and heart rate, but scored by the experts using a set of observation sheets from Cries Neonatal Postoperative Pain Measurement Tool (CNPPMT). Results: During the heel prick, the average of pain response was found higher on sucrose group (2.67). After a minute elapsed, the average of pain response on both groups was similar (1.96). After two and three minutes elapsed, the average of pain response was found higher on swaddling group. Bivariate results showed that there was no different average of the pain response on both groups during the heel prick (p=0.925) and after the heel prick (one minute (p=0.915), two minutes (p= 0.942), and three minutes (p=0.132)). Conclusions: Research revealed that sucrose and swaddling equally effective in reducing pain response on neonatal blood spot by heel prick. Swaddling could be carried out to reduce pain response on infants when pricking applied.

Abstract

Nurse caring behavior is the core or focus in nursing as a form of professional nursing practice and caring behavior is an indicator of patient satisfaction. The purpose of this study was to determine the relationship of nurse caring to the level of patient satisfaction in the inpatient room at Hermina Galaxy Hospital. This study uses quantitative methods. The sampling technique used purposive sampling technique with a sample of 50 respondents who were in accordance with the inclusion and exclusion criteria. The results showed that 32 respondents said caring nurses were good, 19 respondents were satisfied and 13 respondents were dissatisfied. Of the 18 respondents who said that nurse caring was lacking, 7 respondents were satisfied and 11 respondents were dissatisfied. Correlation test results obtained an r value of 0.494 and a p value of 0.000, the conclusion of these results is that the relationship between nurse caring and patient satisfaction shows a positive relationship and has a positive pattern, meaning that the higher the nurse's caring, the higher the level of patient satisfaction. There is a relationship between nurse caring and patient satisfaction in the inpatient room at Hermina Galaxy Hospital.

Abstract

In today's technological developments, wireless communication provides many possibilities to be able to share information with each other anytime and anywhere and in various fields of life such as health services and the industrial world. One of the wireless communication technologies commonly used in health services is WBAN (Wireless Body Area Network), where WBAN (Wireless Body Area Network) can monitor a person's health condition anywhere and anytime by placing sensors around the body. In order for this monitoring process to be more effective and efficient, an application based on Android and IOS systems is designed that can display monitoring results in real time and can be used for patients or medical personnel. In addition, to minimize the performance of medical personnel in diagnosing patient conditions, an automatic diagnosis system is applied by utilizing fuzzy mamdani logic in making decisions from the measurement results obtained on the sensor.

Abstract

Instrumental support is concrete and tangible, for instance, helping to provide food or accompanying patients to health care facilities. However, its impact on medication adherence among tuberculosis patients in Indonesia needs to be explored. This study aimed to examine the impact of instrumental support from family on tuberculosis patients’ adherence to medication in Bogor City, West Java Province, Indonesia. A cross-sectional study involving 106 participants with an age average of 39.7 (SD = 14.9) was conducted in 12 primary health care (PHC). The independent variables (age, sex, education, employment status, wealth index, and the distance between home and PHC) of instrumental support from family was assessed using MMAS-8. In contrast, the dependent variable of medication adherence was assessed using a self-reported instrument. Multivariate binary logistic regression was used in the analysis and indicated that participants receiving family support were more likely to adhere to medication protocol (95% CI = 1.1–6.3; p-value = 0.029). Instrumental support from family was associated with medication adherence among tuberculosis patients in Bogor City, West Java Province, Indonesia. It is necessary to design further comprehensive interventions in the community setting to encourage the family to support tuberculosis patients following medication protocol.

Abstract

Background: Work of breathing (WOB) and non-comorbidities factors in the respiratory system are the two probable findings in pediatric COVID-19 patients. However, the association of those factors with level of care was not well reported.Purpose: This study aimed to identify the relation between potential predictors including comorbidity, low nutritional fulfillment, infectious disease, shock, cough, O2 saturation reduction, abnormal blood gas analysis and sore throat with the level of care among pediatric COVID-19 patients. We also analyzed the synergistic correlation of non-comorbidities factors in the respiratory system and work of breathing to predict level of care in pediatric COVID-19 patients.Methods: A cross-sectional study was conducted in the six referral hospitals from July to September 2020 in four provinces in Indonesia. An observation checklist was used to collect data from the medical records of pediatric patients with COVID-19, including medical diagnosis, demographic, and clinical manifestation. This study included 423 participants aged from 0 to 18. The multivariate logistic regression was performed to test the adjusted odds ratios (AORs) with the 95% confidence intervals (CIs) of the association between WOB, non-comorbid respiratory, and level of care. Moreover, dummy variables (2x2) were made to analyze synergistic correlation of non-comorbid respiratory disease and WOB. The AOR with the 95% CIs was applied in the association between the complication of non-comorbid respiratory diseases and high work of breathing with level of care among pediatric patients with COVID-19.Results: Results showed that age, presence of comorbidity, nutritional fulfillment, infectious disease, shock, work of breathing, O2 saturation reduction, abnormal blood gas analysis, sore throat, and convulsive meningeal consciousness were significantly associated with the level of care (p<0.05). Pediatric patients with non-comorbid respiratory and increased work of breathing had a 15.59 times higher risk of requiring PICU care level (p<0.01). Meanwhile, pediatric patients who experienced both non-comorbid respiratory and increased work of breathing had a 5.76 times risk of requiring an intermediate level of care (p<0.05), and 9.32 times higher risk of requiring a PICU level of care (p<.05).Conclusion: It was found that both non-comorbid respiratory and increased WOB had a significant relationship with the level of care for pediatric patients with COVID-19. Nurse should take into account those clinical findings to increase the awareness in monitoring clinical deterioration in pediatric COVID-19 patients.

Abstract

In this study, a multisensor wireless medical device was designed using several sensors, such as body temperature, oxygen levels, blood pressure, and heart rate, that connects to the application and applies WBAN technology. The design of this tool applied the principle of IoT, where a system that can monitor body health is needed, a remote monitoring system on the smartphone. The data obtained by the tool is sent to the server to be later forwarded to the application. In this study, Fuzzy Mamdani's method was used to implement the health status. The IoT principle makes it effortless for users to access or use pre-designed tools. Therefore, in this study, the IoT principle is employed to make it more efficient and easier for users to read the results because the data will be forwarded to applications installed on users' smartphones, both Android and iOS-based.

Abstract

Chronic kidney failure is a progressive and irreversible renal function disorder in which the body's ability to fail to maintain metabolism and fluid and electrolyte balance. Patients undergoing long-term hemodialysis must be faced with various problems such as financial problems, difficulty in maintaining a job, depression and fear of death that affect their quality of life. The purpose of this study was to describe the quality of life of patients with chronic kidney failure undergoing routine hemodialysis therapy at RSHY. The research method is descriptive with a cross sectional approach. The population is 70 patients with chronic renal failure who undergo routine hemodialysis therapy in the Hemodialysis Room RSHY according to the inclusion criteria and exclusion criteria determined by the researcher. The results showed that the quality of life was seen from 4 aspects, namely physical, psychological, social and environmental. From a physical point of view, the majority of 47 (67.1%) have a moderate quality of life. In terms of psychology, the majority there are 50 (71.4%) respondents have a moderate quality of life. From a social perspective, the majority of respondents have a moderate quality of life as many as 47 (67.1%). In terms of the environment, the majority of 36 (51.4%) respondents have a moderate quality of life. The conclusion of this study is that there is a significant difference in the quality of life of younger CKD patients undergoing hemodialysis on the physical aspect. In addition, there were also significant differences in quality of life in patients using HD Cath vascular access on psychological aspects.

Abstract

Operasi sesarea merupakan metode persalinan yang banyak dipilih saat ini. Hal ini menyebabkan nyeri dan mempengaruhi jumlah prolaktin darah serta motivasi ibu untuk menyusui bayinya. Oleh karena itu diperlukan penanganan nyeri pasca persalinan agar proses menyusui dapat berjalan dengan optimal. Relaksasi Benson merupakan terapi komplementer yang dapat dilakukan untuk mengurangi nyeri. Tujuan Penelitian ini adalah untuk mengetahui pengaruh teknik relaksasi Benson terhadap intensitas nyeri dan menyusui pada ibu post seksio sesaria di Rumah Sakit Hermina. Metode: Penelitian ini menggunakan desain quasy eksperiment yang dibagi menjadi dua kelompok yaitu kelompok intervensi dan kontrol, jumlah responden sebanyak 80. Penelitian dilakukan di Rumah Sakit  Hermina pada bulan Oktober-Januari 2022. Uji statistik yang digunakan dalam penelitian ini adalah Mann -Whitney. Hasil penelitian ini rata-rata intensitas nyeri pada kelompok intervensi setelah penerapan teknik relaksasi Benson adalah 2,03(0.791) dan pada kelompok kontrol 3,95(0.832) dengan P Value = 0.000,  sedangkan distribusi pemberian ASI eksklusif P Value=0,31 tidak memiliki perbedaan yang significan. Intervensi relaksasi Benson diharapkan dapat digunakan sebagai bahan pertimbangan dalam mengembangkan metode  nonfarmakologi komplementer untuk menurunkan intensitas nyeri pada pasien post seksio sesarea yang dirawat di rumah sakit.

Abstract

This research proposes a multifunctional wireless health monitoring tool with a display for Android and iOS devices. This research aims to develop a realistic solution for real-time and conveniently accessible health monitoring via mobile devices. The device allows users to test and track health factors such as heart rate, blood pressure, blood oxygen levels, body temperature, and blood glucose. It collects data properly by using wireless technology and sensors. The data is subsequently supplied to the appropriate apps on Android and iOS devices. The data is presented visually in the program, making it instructive and user-friendly. The device's development technique involved extensive testing and validation against established comparators to assure accuracy. The results of this study show that this digital, multi-purpose health monitoring device works well and reliably to give real-time health information. This innovation promotes health monitoring and digital health information access.

Abstract

Chronic Obstructive Pulmonary Disease (COPD) is a lung disease which has a chronic obstruction in airway that can interefere normal respiration led to death. Patients with COPD usually show symptoms such as dyspnea that are most frustrating in patients with COPD that can trigger the appearance of tightness and decreased saturation. The semi-fowler position is a position that can be chosen by patients who have difficulty breathing and in COPD patients is expected to optimize lung development so that oxygen saturation becomes increased. Given the many factors that contribute to oxygen saturation in patients, it is important to control the factors that affect oxygen saturation in patients who get a semi-fowler position. In addition, given the high prevalence of COPD, it is important for hospitals to identify factors that affect the length of day of treatment of patients with COPD. Identification of these factors can help health workers to plan treatment programs to plan the length of the day of treatment of patients who enter the hospital. The study aimed to identify factors that influence oxygen saturation and the length of days of treatment in COPD patients who get semi-fowler positions at Hermina Ciputat Hospital. Data collection was carried out from January 2020 to October 2021 using the cross-sectional method, namely by taking data at once at the same time through medical record data on 84 inpatients. The analysis test used in this study is the T-test and subsequently multivariate analysis is carried out using linear regression tests. Based on the analysis test, the factors that affect oxygen saturation are gender, smoking history, and initial saturation value. While the factors that affect the length of treatment are the history of smokers and the history of comorbid. Respondents who had a history of smoking would experience a decrease in oxygen saturation by 0.3888. Preventive interventions through smoking-related health education need to be done to reduce the severity in patients with COPD who are hospitalized.
