Abstract

Hyperbilirubinemia in neonates is often a health problem that can cause serious complications such as encephalopathy, kernicterus, and even death if not treated properly. Currently, treatments focus on phototherapy, which despite its positive impact, also has the potential to cause negative effects such as diarrhea, dehydration, skin rash, blue baby syndrome, and skin darkening. Massage therapy is emerging as a potential intervention to reduce the need for phototherapy and lower bilirubin levels. Although massage therapy shows a positive impact, it has not been proven to be fully effective in lowering bilirubin levels in neonates with hyperbilirubinemia. The aim of this systematic review is to assess the effectiveness of massage therapy on reducing bilirubin levels in neonates with hyperbilirubinemia. In this review, 127 articles were selected, and the results showed that massage therapy was effective in reducing bilirubin levels in neonates. In conclusion, massage therapy can be implemented in health services as an effective approach to treat hyperbilirubinemia in neonates.

Abstract

Aims: Controlling interdialytic weight gain (IDWG) in hemodialysis (HD) patients requires a key component of fluid restriction. If IDWG rises, complications related to chronic renal failure may arise. Digital self-management improves fluid restriction in IDWG by empowering patients, increasing data monitoring, and simplifying communication with healthcare providers, perhaps leading to better hemodialysis patient outcomes. Objective: This study aims to determine the effectiveness of digital self-management toward fluid restriction on interdialytic weight gain among patients undergoing hemodialysis in Jakarta Islamic Hospital. Method: The study was quasi-experimental, using a pretest-posttest and control group. Convenience sampling was employed to select 25 intervention and 25 control group members. IDWG digital self-management can track fluid intake, weight, and symptoms, and patients can communicate openly with the health team. The data analysis used in this research is the T-test.  Results: In this study, respondents were categorized by gender, mean age, and duration of HD (months) in the intervention group (men 60%, 49.1 and 27.2) and control group (women 52%, 48.1 and 42.1). IDWG decreased in the intervention group, from 4.06 to 2.45, while it increased in the control group before 3.84 and after 4.34. The paired and independent t-tests demonstrated that digital-based fluid restriction management self-care interventions reduced IDWG with a p-value of 0.001. Conclusion: The digital self-management toward fluid restriction showed a promising impact on improving interdialytic weight gain among patients undergoing hemodialysis in Jakarta Islamic Hospital

Abstract

ntroduction: Nurses play a pivotal role in the provision of healthcare services both within the Indonesian context and on a worldwide scale. The development of nursing competence is crucial in order to guarantee the provision of high-quality healthcare services. To examine the elements that are linked to the enhancement of nurse competence by means of the adoption of nursing career paths within hospital settings in Indonesia. Methods: A descriptive analytical design, utilising a cross-sectional approach with a sample size of 197 nurses. The data was obtained by administering a meticulously evaluated questionnaire, designed by the researchers, to the participants. The questionnaire's validity (r=0.361) and reliability (r=0.933) were thoroughly assessed. The analysis of the research data was conducted utilising multiple linear regression tests. Results: As established through the use of multiple linear regression tests, demonstrate a statistically significant positive association (p<0.05) between organisational loyalty, organisational support, guidance, and financing, and the implementation of nurse competence quality. The obtained coefficient of determination was 0.260, suggesting that the four independent variables considered in this study have the ability to collectively account for 26% of the variability observed in the implementation of nurse competence quality. It is important to note that the remaining variation is likely impacted by additional factors not included in the analysis. The guiding element has been identified as the primary determinant in the cultivation of quality competence within hospital settings while implementing nursing career routes. Conclusion: The present study establishes that factors such as organisational loyalty, organisational support, mentorship for nurses, and financial resources for career advancement have a significant impact on the enhancement of nursing competency along the career trajectory within hospital settings. The variable exerting the most significant influence is the provision of instructions for nurses.

Abstract

Background: The challenge of fatigue significantly affects children undergoing cancer treatment, impacting their overall well-being and quality of life. Unlike their healthy counterparts, pediatric cancer patients grapple with the demanding impacts of chemotherapy, leading to anemia and nutritional deficiencies that exacerbate fatigue. Beyond fatigue, chemotherapy alters appetite, nutrient utilization, and mental function. Fatigue's multidimensional impact, marked by reduced energy and increased need for rest, can detrimentally affect the quality of life for pediatric patients. Understanding these challenges is essential for devising tailored strategies that comprehensively address the unique needs of children undergoing cancer treatment, enhancing their overall well-being and quality of life. Objective: This research aims were to determine the relationship between anemia and nutritional status with the occurrence of fatigue in children with cancer undergoing chemotherapy at Dharmais Cancer Hospital in Jakarta. Methods: An analytical cross-sectional study conducted in August 2022 included a sample of 24 children selected through accidental sampling. Demographic and disease characteristics, such as age, gender, cancer type, cancer stage, and treatment duration, were collected through a questionnaire. Anemia was determined by hemoglobin levels (12 mg/dl), and nutritional status was assessed using Body Mass Index. Fatigue was measured with tailored instruments for different age groups: the Parent Fatigue Scale (PFS) for children under 7 years and the Child Fatigue Scale (CFS) for those aged 7 years and older. Statistical analyses included univariate assessment, bivariate analysis with the chi-square test, and logistic regression. Results: Demographic analysis revealed a predominantly male sample (70.8%) with a significant adolescent presence (41.7%) and a prevalence of leukemia (37.5%). Most participants were in early cancer stages (58.3%), undergoing treatment for over a year (62.5%). Anemia was common (54.2%), and 58.3% were underweight. High fatigue prevalence was observed (62.5%). Chi-square analysis found no significant gender or cancer stage differences in fatigue. Age and treatment duration showed no significant associations with fatigue. Logistic regression revealed a significant association between anemia and fatigue in the first model (p = 0.022, Exp. B = 0.104). The second model, incorporating nutritional status, showed a non-significant relationship with anemia but a notable association with normal nutritional status and fatigue (p = 0.022, Exp. 0.059). This underscores the crucial role of nutritional interventions in mitigating fatigue. Conclusion: This study highlights a significant relationship between anemia and nutritional status with fatigue in children undergoing chemotherapy. These findings underscore the importance of considering anemia and nutritional interventions when providing care to pediatric cancer patients undergoing chemotherapy. Nurses play a vital role in addressing these concerns and enhancing the overall well-being of children with cancer.

Abstract

Tuberculosis (TB) is a global health challenge that significantly impacts individuals and communities. Family-based care is recognized as an effective method to enhance TB management. The aim of this case study is to illustrate the implementation of family-based nursing care in a TB case to provide holistic care. The method used is a case study approach following the Family Nursing Model (FCN) theory by Friedman. A family with TB-diagnosed members was selected as the sample. The nursing care process was conducted over three months with bi-weekly routine visits. The care process included assessment, diagnosis, intervention, implementation, and evaluation stages. The results of this case study formulated the nursing diagnosis “Ineffective Health Maintenance in the Family of Mr. D”. Interventions were based on the five family health tasks, namely understanding health issues through education about TB and the disease process; making informed decisions involving the family; providing care with support for the treatment program undergone by family members; adapting a healthy environment through hygiene behavior education; and utilizing health services through referral to community services. This nursing care illustrates integrated care practices with active family involvement, providing practical guidance to enhance TB treatment outcomes and management at the family level.
