Abstract

Introduction: The Emergency Department (ED) is one of the first accesses reached by people affected by Covid-19 or suspected of having Covid-19. Changes in health services in the ED during the Covid-19 Outbreak and the adaptation of the new normal made ED health services and adjustments by nurses. The ED is one area that has the potential to have a high risk of transmitting Covid-19. Objectives: This study aimed to determine the experience of emergency room nurses in dealing with the Covid-19 outbreak at a government hospital in Padang. Methods: This study is qualitative with semi-structured interviews conducted with nurses who serve in the ED, who were selected based on inclusion criteria. These nurses had served in the ER during the Covid-19 outbreak for a minimum of 6 months. The results of the interviews were processed using the Collaizi technique. The participants in this study were 5 emergency room nurses. Results: The results obtained 3 themes and 11 sub-themes. The themes that can be found are Nurses' Preparation for the Pandemic, Coordination and Health Promotion with Families, and Hospital and Emergency Room Readiness. Conclusions: Hospital management have to prepare and coordinate human resources and facilities following non-natural disaster conditions so that the nurse's response ability in dealing with the Covid-19 pandemic becomes responsive and better.

Abstract

Introduction: Professional nursing caring behaviors can be learned through education. Indonesian research on this area remains limited. This study explores the development of nursing students' professional caring behaviors and clinical learning processes that support them. Method:This ethnographic study involved 34 observations of nursing student clinical practices, field notes, and six focus group discussions with students and clinical instructors from three nursing education institutions across three provinces. Data analysis was conducted using qualitative content analysis. Result: Ten themes revealed: professional nursing caring behaviors are understood as altruistic behaviours, holistic, therapeutic, and bureaucratic dimensions; instrumental caring predominates in student clinical practices; physical interventions serve as the entry point for communication; the provision of information is dominant in interpersonal communication; cultural sensitivity and touching are expressions of caring behaviors; clinical role models, knowledge, and experience are vital in developing professional caring behaviors; patient negative responses, clinical workload, and patient complexity pose challenges in developing professional nursing caring behaviors; insufficient clinical mentoring impacts the achievement of competencies; students' lack of self-confidence is perceived as 'uncaring' behaviors; and there is insufficient collaboration to ensure the attainment of non-instrumental caring competencies. Conclusion: Students' competencies in professional caring behaviors evolve during their educational journey, though the focus remains on instrumental caring. Knowledge, clinical exposure, experiences, role models, and self-confidence are crucial in enhancing professional nursing caring competencies. Clinical learning guidelines, such as preceptorship and collaborative learning models in professional nursing caring competencies, should be considered to develop the skills.

Abstract

Salah satu indikator   mutu pelayanan kesehatan di ruang rawat inap adalah meningkatnya angka kenyamanan pasien atau nyeri terkontrol. Pengetahuan dan sikap yang akurat dalam penerapan manajemen nyeri merupakan prinsip penting  dalam  praktek  keperawatan  karena  berdampak  langsung  terhadap pasien.Salah satu upaya untuk meningkatkan pengetahuan dan sikap perawat mengenai  manajemen  nyeri  adalah melalui  pelatihan.  Tujuan  penelitian  ini untuk menganalisis pengaruh pelatihan manajemen nyeri bagi perawat terhadap pengetahuan, sikap dan penerapan manajemen nyeri di IRNA Non Bedah Paru dan Saraf RSUP Dr. M. Djamil Padang. Penelitian ini menggunakan desain quasi-eksperiment  dengan pendekatan one group pre-post test design. Sampel penelitian  ini  adalah  perawat  pelaksana  yang  berjumlah  30  orang  diambil secara  total  sampling.  Penelitian  ini  menggunakan  kuesioner  dan  lembar observasi   yang   sudah   diuji   validitas   dan   reliabilitasnya.   Analisa   data menggunakan uji wilcoxon. Hasil penelitian menunjukkan terdapat peningkatan pengetahuan,  sikap,  dan  penerapan  manajemen  nyeri  oleh  perawat  setelah pelatihan  secara  signifikan  (p<0,005).  Pelatihan  secara  berkelanjutan  dan evaluasi    penerapan    mengenai    manajemen    nyeri,    diharapkan    dapat meningkatkan kemampuan perawat dalam melaksanakan manajemen nyeri.

Abstract

Neonatal mortality is a leading cause of infant deaths, particularly in developing countries. In Serdang Bedagai Regency, the coverage of neonatal visits remains low, contributing to the high neonatal mortality rate and causing economic losses. This study aims to identify factors related to the neonatal visit coverage rate in Serdang Bedagai Regency, North Sumatra, Indonesia. This study used a cross-sectional design with multivariate analysis. Data were collected from 200 mothers with babies aged 0-28 days in five health centers in Serdang Bedagai Regency. The variables analyzed include age, education, economic status, knowledge, maternal attitudes, culture, family support, midwife attitudes, access to health facilities, and service quality. The data were analyzed using logistic regression. The results showed that the variables consistently and significantly affecting the increase in neonatal visit coverage were maternal education, knowledge about neonatal visits, local culture, husband/family support, and the quality of visit services. Factors that significantly contributed to increasing coverage of neonatal visits were maternal education (p-value = 0.004, OR = 14.285), maternal knowledge (p-value = 0.000, OR= 75.920), local culture (p-value = 0.000, OR = 57.105), husband/family support (p-value = 0.000, OR = 77.763), and service quality (p-value = 0.000, OR = 47.474). These factors have a major influence in supporting the success of the neonatal visit program, with maternal knowledge and family support being the most dominant factors.. Efforts to increase coverage should involve educating mothers, involving families, and enhancing services while considering local wisdom.

Abstract

Pelayanan keperawatan terus dituntut untuk memberikan pelayanan terbaik dan profesional. Instansi pelayanan berusaha memperkerjakan personil yang dapat memenuhi kriteria profesional seperti yang diharapkan, yaitu lulusan perawat profesi (ners). Seiring dengan hal ini, permasalahan lain yang muncul adalah bagaimana ners fresh graduate dengan minimal pengalaman kerja memenuhi tuntutan pekerjaan saat pertama kali bekerja. Penelitian ini bertujuan menggali tantangan lulusan ners fresh graduate saat bekerja pertama kali di rumah sakit. Dengan menggunakan desain penelitian kualitatif dan pendekatan studi fenomenologi, perawat yang bekerja di sebuah Rumah Sakit Pemerintah di Kota Padang, Sumatera Barat, dipilih seabgai partisipan dalam penelitian ini berdasarkan teknik purposive sampling. Didapatkan enam perawat yang berlatar belakang pendidikan Ners fresh graduate yang diwawancarai mengenai pengalaman mereka bekerja pertama kali di rumah sakit dengan menggunakan metode in-depth interview. Rekaman wawancara disalin menjadi bentuk transkrip verbatim, kemudian dilakukan diektraksi menjadi kategori dan subkategori. Penelitian ini mendapatkan tiga kategori, yaitu merasakan beban peran dan tanggung jawab yang besar, masalah komunikasi dengan sejawat, dan menemukan gap antara yang dipelajari di perkuliahan dengan yang dihadapi di rumah sakit. Diharapkan manajemen keperawatan di rumah sakit dapat menguatkan program orientasi bagi ners fresh graduate agar bisa menghadapi tantangan kerja saat bekerja pertama kali di rumah sakit.

Abstract

The educational and socialization activities conducted on September 13, 2023, aimed to increase community knowledge about diarrhea and its home management. Diarrhea is a common health problem among children under 3 years old in developing countries, with an average of 3 episodes per year. Each episode of diarrhea can cause children to lose the necessary nutrients needed for growth, making it the leading cause of malnutrition in children and the second leading cause of death after pneumonia. One recommended dietary method for managing acute diarrhea is the BRAT Diet, which consists of Bananas, Rice, Applesauce, and Toast. This diet helps reduce the workload on the digestive system and provides easily digestible foods. This educational activity targeted mothers with toddlers in the working area of Pauh Community Health Center. The program was implemented through counseling using PPT media and distributing leaflets to the participants. The activity was evaluated using pretests and posttests to measure the increase in community knowledge. The evaluation results showed an increase in community knowledge after the educational and socialization activities were conducted. This program highlights the importance of education and socialization in enhancing community knowledge about home management of diarrhea, which can ultimately help reduce the rates of malnutrition and child mortality.

Abstract

Background: Nursing services are often used as a benchmark for the image and quality of a hospital in the eyes of the community. One indicator of improving the quality of service is job satisfaction of nurses. Nursing round is one of the activities that can increase nurse job satisfaction. This study aims to identify the effect of the implementation of the nursing round on the job satisfaction of implementing nurses in the inpatient ward. Methods: This type of research uses a pre-experimental design with the One Group Pretest-Postest Design which was carried out in May-July 2022 with a population of all nurses in the inpatient room of RSI Ibnu Sina Padang with a sample size of 30 respondents. Instruments in this study in the form of questionnaires and observation sheets. Results: The results showed that there was a significant effect of the nursing round on the level of job satisfaction of nurses in the inpatient room with a p-value of 0.022 (p <0.05). There was a significant difference in the level of job satisfaction of nurses before and after the nursing round (p < 0.05). Conclusion: It is hoped that hospital management will measure the job satisfaction of implementing nurses and develop policies related to the implementation of the nursing round.

Abstract

Background: Clinical learning has a very important role in determining the quality of nursing students' learning achievements, because clinical knowledge and skills are the main foundation in clinical education. Nursing students are expected to have competency in the knowledge and skills to provide high quality and safe nursing care to patients, however there are still many nursing students who feel they are not ready to become nurses in the future because they feel they are not yet competent in carrying out nursing care to patients. The aim of the research is to see the correlation between self-efficacy and the clinical competence of Nurse Professional Education students. Methods: This research used correlation research and the sample in this research were clinical practice students of the Nursing Professional Education who had undergone 2 clinical learning stages in semester 1 which were taken using a total sampling technique. Data analysis using SmartPLS. Results: From the research results, it was found that self-efficacy had a positive effect on clinical competence (P Value 0.000, T Statistics 14.273, sample mean 0.686). Self-efficacy influences clinical competence by 46.3% (R Square 0.463), the remainder is influenced by other factors not studied. Conclusion: These findings emphasize the importance of self-efficacy in forming and improving students' clinical competence. By having high self-confidence in their abilities, students are more likely to be able to apply their knowledge and skills effectively in clinical practice.

Abstract

Outpatient waiting time is one of the national quality indicators. Most patients visiting outpatient clinics in Indonesian hospitals complain about the long waiting time before receiving healthcare services. Over the decades, the persistent issue of long waiting times and unresolved patient dissatisfaction has led hospitals worldwide to adopt Lean Healthcare management systems. These systems focus on improving the efficiency and effectiveness of service operations by eliminating waste. Siti Rahmah Hospital is a private type-C hospital facing outpatient waiting time issues exceeding 60 minutes. This community service aims to enhance nurses' knowledge and reduce service waiting times. This community service was conducted using a quantitative approach with an experimental design, and the primary data collection technique was through questionnaires. The implementation consisted of four main stages: preparation, socialization of lean healthcare methods, identification of waiting times, and mapping types of time waste in services. Nurses' knowledge about lean healthcare management increased significantly by 56.70% after the training, as shown by a pretest score of 36.6% and a post-test score of 93.3%. The knowledge components that improved substantially included understanding the outpatient waiting time standard of less than 60 minutes, identifying time-wasting activities in services, and using electronic medical records as a lean healthcare strategy to reduce service waiting times. The lean healthcare method is effective in reducing patient waiting times in outpatient care.
