Abstract

 The Health-Related Quality of Life (HRQoL) is one of the important psycho-social characteristics that can affect patient’s ability to manage therapy. Poor of knowledge of hypertension and the changing lifestyle can affect the quality of life of patients. One of the pharmacist’s interventions in hypertension management is to conduct counseling. Motivational counseling helps health service to assess patient’s understanding and patient’s readiness to change patient’s behavior. Some motivational counseling methods still need to be developed. Therefore, this study aims to find the influence of the "brief counseling-5A" and "motivational SMS" by a pharmacist on the quality of life and blood pressure control in hypertension patients in the internal disease polyclinic, PKU Muhammadiyah Bantul Hospital, Yogyakarta. The study has been done by using the quasi-experimental method with prospective data collection during the period of January until April 2013. Sixty patients have met inclusion criteria and were divided into two groups. Thirty patients (50%) received "brief counseling-5A" and "motivational SMS" as intervention group and the other thirty patients (50%) received usual care as a control group. The data collection was done by interviewing patients. Medication adherence and QoL were assessed by using Morisky Medication Adherence Scale (MMAS) and SF-36. The values of blood pressure are taken from patient’s medical records. Patient’s quality of life showed a good improvement during post study. It is shown in 8 different domains including pain, fatigue, physical function, emotional function, social function, role physical, mental health, and general health. In intervention group, physical function, emotional function, and pain showed highly significant improvement (p<0.05). Systolic and diastolic blood pressure in the intervention group decreased significantly (p<0.05) (systolic p=0.001 and diastolic p=0.018) in the post study. From this study, it is concluded that the "brief counseling-5A by pharmacist" and "motivational SMS" had a positive impact on patient’s QoL and patient’s blood pressure, hence, the therapy outcome has been achieved.

Abstract

Prevalence of Diabetes Mellitus (DM) with Hyperlipidemia increased in Indonesia, where both diseases have a risk of complications towards cardiovascular disease. DM patients with hyperlipidemia require proper management to control blood sugar, total cholesterol and LDL cholesterol. Knowledge is one of the important factors in improving adherence therapy and quality of life of DM-hyperlipidemia patients, especially in controlling blood glucose level and total cholesterol levels. This study aims to develop a patient questionnaire of DM-hyperlipidemia.The study used cross sectional method. Inclusion criteria were patients aged 18 – 70 years old, diagnosed DM-Hyperlipidemia, getting at least 1 oral antidiabetic and oral anti-cholesterol. The questionnaire item of knowledge was developed based on gudeline therapy and previous research reference. Expert validation is done on Doctor who had expert in DM and clinical psychologist. The pilot test was performed on 10 healthy patients and 10 patients of DM-hyperlipidemia who had eligible inclusion criteria. Validation of questionnaire was done on 30 patients of DM-Hyperlipidemia who undergoing therapy at outpatient clinic of PKU Muhammadiyah Hospital Yogyakarta. Data collection was done by interviewing questionnaires in patients. Analysis of data using SPSS with product moment validation test and cronbach alpha to see the reliability of questionnaire.Validation results show there are some invalid items that are in questions 9, 11, 12, 15, 16 and 17, the range of questions about knowledge about hyperlipidemia, medication and general health. Re-validation without invalid item was done and obtained value r&gt; 0.3, with value of Cronbach alpha 0,715.The DM-Hyperlipidemia patient questionnaire was valid and reliable to measure the level of knowledge of DM patients with Dysipidemia.

Abstract

Sekolah berbasis asrama memiliki beberapa permaslahan terutama di bidang kesehatan. Hasil survey dan skrining kesehatan yang telah dilakukan menunjukkan tingginya prevalensi penyakit menular seperti flu, batuk pilek (16.5%) dan penyakit kulit seperti jamur, gatal, scabies (11.2%). Adanya pos kesehatan pesantren di lingkungan sekolah tersebut diharapkan dapat mengoptimalkan promosi kesehatan dalam upaya pencegahan penyakit. Hal tersebut didukung pula dengan optimalisasi pemanfaatan TOGA (tanaman obat keluarga) yang ada di setiap asrama sekolah. Lahan yang cukup luas dan tempat tinggal (asrama) siswa yang terpisah menjadi peluang dalam pengembangan penanaman TOGA, di dukung dengan pemanfaatan TOGA dalam bentuk sediaan minuman kesehatan dirasa lebih menarik dan penting untuk diupayakan sebagai salah satu kegiatan edukasi dan promosi kesehatan di lingkungan sekolah berasis asrama. Kegiatan pemberdayaan masyarakat ini bertujuan untuk melakukan pendampingan pemanfaatan TOGA dan pelatihan pemanfaatan tanaman Jahe (Zingiber officinale) dan tanaman rosella (Hibiscus sabdarifa) sebagai minuman kesehatan (syrup). Kegiatan ini meliputi dua sesi, yaitu sosialisasi hasil skrining kesehatan siswa dan edukasi kesehatan diri “swamedikasi” yang diikuti oleh pengelola madrasah, staf poskestren (perawat dan dokter), serta ibu pamong asrama (ummahat). Adapun kegiatan kedua adalah pelatihan pemanfaatan TOGA  Jahe (Zingiber officinale) dan Rosella (Hibiscus sabdarifa) sebagai minuman kesehatan yang diikuti oleh 44 siswa Madrasah Mu’allimin Muhammadiyah Yogyakarta. Pengetahuan siswa sebelum dan sesudah pelatihan menunjukkan peningkatan skor rerata dari 5.56 menjadi 9.18 dengan nilai signifikansi P=0.000 (P<0.05). Hal ini menunjukkan bahwa pelatihan ini efektif dan dapat meingkatkan pengetahuan siswa Madrasah Mu’allimin Muhammadiyah Yogyakarta. Melalui pelatihan ini diharapkan siswa mampu mengaplikasikan terutama saat progam Mubaligh Hijrah yang menjadi salah satu program rutin Madrasah dalam praktek di masyarakat sekitar. Kegiatan ini diharapkan pula dapat ditindak lanjuti oleh poskestren untuk bersinergi dengan ibu pamong ataupun kantin untuk dapat mengoptimalkan TOGA di lingkungan asrama dan memulai produksi sediaan syrup kesehatan Jahe dan Rosella.

Knowledge and adherence are important factors in the treatment of DM-dyslipidemia either to control blood glucose and cholesterol or to prevent from complications. Education is one of the ways to increase knowledge and adherence. The aim of this research is to determine the impact of intervention (Patient Information Leaflet (PIL) and motivation messages by SMS) to knowledge and adherence of DM-dyslipidemia patients in PKU Muhammadiyah Yogyakarta Hospital. This research is a quasi experimental study with analytical quantitative. The data taken from February 2016 to April 2017 at PKU Muhammadiyah Yogyakarta Hospital. Inclusion criterias in this research are patient with age more than 18 years old, with diagnosed by DM-dyslipidemia, using 1 or more of DM and dyslipidemia medicine, not deaf, and cooperative. Interventions that given to the patients are Patient Information Leaflet (PIL) and motivation messages by SMS which routinely given in a week after first we met. Knowledge is assessed using questionnaire and adherence is assesed using pill count method. The data that need analyzed are characteristic patients and the impact of intervention using Wilcoxon and paired t-test. The result of intervention in 56 patients showed the increasing number of patient that have good knowledge after intervention, from 58,9% to 71,4%, otherwise it not differ significantly (p 0.071). Other than that, the adherence also increased that showed from the patients who have adhere category after intervention, from 58,93% to 91.07% and it differ significantly with p 0.001 (p<0.05). Abstract

Diabetes mellitus and dyslipidemia are non communicable disease which those prevalence increased significantly.Those are risk factors of cardiovascular disease. A good knowledge is important to enhance the awareness of disease, so that the target of outome therapy can be achieved. The aim of this research are to describe the patient?s knowledge about the disease and to know the relationship between patient?s knowledge and outcomes therapy (HbA1c and total cholesterol). This research is an observational analytic study with cross setional design. The data taken prospectively from diabetes patients in PKU Muhammadiyah Hospital around September to December 2016. Sample who met the inclusion criterias were patient who diagnosed with diabetes mellitus-dyslipidemia, aged over 18 years old, have a HbA1c and total cholesterol data and using minimal 2 drugs, antidiabetic and anticholesterol. The datas are the knowledge data which taken using validated questionnaire and the outcomes therapy which taken from patient?s medical data. Linear regression analysis is done to know the relationship between knowledge and outcome therapy. Around 37 patients who participated in this research were below 60 years old (64.9%), mostly female (59.5%), and have en education more than 9 years (86.5%). Besides that, only 62.2% patient who achieved the target of HbA1c, and only 56.8% patient who achieved the target of total cholesterol. Furthermore, based on linear regression analysis showed that there was no relationship between knowledge and outcome therapy either HbA1c or total cholesterol (sig 0.893 and 0.292).

Abstract

Antihypertensive therapy requires adherence therapy to achieve therapeutic targets such as controlling blood pressure both systolic and diastolic blood pressure. Pharmacists in counseling interventions have been developed in the form of brief Counseling - 5A and SMS reminders. But the relationship between adherence and blood pressure control is still studied and both of them are related to many factors that can affect. The aim of this study was to determine the relationship of the level of compliance antihypertensive therapy and control of blood pressure (systolic and diastolic) hypertension patients in outpatient PKU Muhammadiyah Bantul, Yogyakarta who get Brief counseling - 5A and SMS motivation.Patients who met the inclusion criteria were divided into 2 groups, the treatment group (n = 30) who received Brief counseling - 5A and SMS motivation, and the control group (n = 30) who received usual care (conventional counseling). Data were collected by interviews using questionnaires compliance of Morisky Medication Adherence Scale (MMAS), the value of blood pressure is taken from the medical record. Correlation between level of adherence and blood pressure control have been analyzed using Fisher test. The results showed the treatment group with categories adherent patients (n=25) was increase in blood pressure control by 36.67% compared with the control group of patients adherent category (n=6) only 6.67% that has control of the blood pressure on post study. In contrast to patient with adherent category, who had uncontrolled blood pressure are 46.67% (treatment group) compared to the control group 13.33% on the post study. Patients categories of non-adherent but is having control of blood pressure showed 3.33% (treatment group) and 26.67% (control group) on the post study. Although the rate of compliance in the treatment group was higher than control group on the second visit, but there was no significant relation between the level of compliance and controlling blood pressure both in control group (p=1.000) and intervention group (p=0.622) on the second visit (post). The rate of compliance and blood pressure control of patients with hypertensive was low. There were many factors such as poor of knowledge, drugs side effect, poly pharmacy, which related with level of compliance. There was no correlation between antihypertensive compliance and blood pressure control in hypertensive patients ambulatory PKU Muhammadiyah Bantul Hospital, who get Brief counseling 5A and SMS motivation. Therefore it is necessary to do further research to identify factors that influence to improve patients awareness in the management of hypertension.

Abstract

This study aimed to develop and validation a questionnaire as measurement instrument for knowledge and adherence behavior of DM patients in primary health care. Cross sectional study design was conducted in diabetes mellitus patient. Inclusion criteria were patients in the age group 18-65 years, diagnosed with DM, receiving at least one oral antidiabetic medication. Questionnaire questions for behavioral item were developed based on Diabetes Mellitus management guidelines and references to previous studies. Evaluation and validation by expert was carried out on diabetes mellitus experts and clinical psychologists. The pilot study was conducted on 10 healthy patients and 10 patients with diabetes who enrolled inclusion criteria. Questionnaire validation test was conducted with 41 DM outpatient at PKU Muhammadiyah Hospital in Yogyakarta. Collecting data by interviewing patients based on questionnaire. Statistical analysis was performed using SPSS with Pearson correlation coefficients for validation test and Cronbach alpha coefficients for reliability test of the questionnaire. Adherence behavior questionnaire consists of 12 question items, which are divided into three domains: cognitive, affective, and psychomotor domains. Validation results showed 12 valid items where the pearson correlation value was>0.308 (n=41). Cronbach alpha as reliability test results showed 0.78. This result showed a questionnaire were valid and reliable in Diabetes Mellitus patients. This instrument would be use in primary health care for measuring adherence behavior of DM patients.

Abstract

Knowledge of diabetes mellitus (DM) management can improve adherence in medication-taking behavior. Counseling by pharmacists is believed to contribute to its better acquisition. Nevertheless, many counseling and reminder media are still currently developed. This study aimed to identify the effects of brief counseling-5A modification and a motivational reminder through short message services (SMS), designed with a religious approach, on medication-taking behavior and adherence among DM outpatients at Jetis 1 Community Health Center, Bantul, Yogyakarta, Indonesia. This quasi-experimental study employed a pretest-posttest control group design. Data were collected prospectively from May to August, 2018. There were 72 patients who met the inclusion criteria divided evenly into two groups, the treatment group and the control group. The data collected by interviews using questionnaires on medication-taking behavior and the medication adherence rating scale (MARS) and were statistically assessed using the Wilcoxon test. The results showed that brief counseling-5A, combined with religious motivational SMS, raised the stage of behavior change in DM patients. There were increases (Δ) in medication-taking behavior by 2.76±1.59 and patient compliance by 3.44±3.45. In conclusion, brief counseling-5A by pharmacists and motivational text reminders can improve the medication-taking behavior and compliance of DM patients with the therapy.

Abstract

ABSTRAK Anemia pada kehamilan merupakan salah satu masalah dalam kehamilan. Data WHO pada tahun 2001 menunjukkan bahwa lebih dari 50 % wanita hamil mengalami anemia. Anemia pada kehamilan merupakan suatu kondisi dimana kadar hemoglobin kurang dari 11 mg/dL. Prevalensi anemia di Indoensia cukup tinggi yaitu sebesar 50-63%, sedangkan prevalensi di DI Yogyakarta adalah 14,32%.  Penelitian mengenai pengetahuan ibu hamil terkait kondisi kehamilannya di Selangor  diperoleh bahwa pengetahuan ibu hamil pada kategori baik, namun sebagian besar dari subyek memiliki penegtahuan yang rendah terkait suplemen zat besi. Instrumen penelitian mengenai pengetahuan dan suplemen zat besi yang tervalidasi di Indonesia masih belum ada, sehingga data pengetahuan ibu hamil mengenai kehamilan dan suplemen zat besi masih sangat kurang. Metode dalam penelitian ini adalah dengan melakukan pengembangan instrumen penelitian (kuesioner) yang dilakukan validasi oleh tiga orang ahli di bidangnya masing-masing (validasi konten). Langkah selanjutnya adalah dengan melakukan pilot test untuk mengetahui masukan maupun umpan balik dari subyek terkait kesalahan dan item pertanyaan yang membingungkan. Pilot test juga digunakan untuk mengukur waktu pengisian kuesioner. Langkah terakhir adalah melakukan validasi construct yang dilakukan pada 49 subyek penelitian. Analisis statistik menggunakan pearson correlation  dan cronbach alpha digunakan untuk mengukur validitas dan reliabilitas kuesioner. Subyek penelitian dalam validasi ini adalah sebanyak 49 pasien hamil yang mendapatkan suplemen zat besi dan atau asam folat di RS PKU Muhammadiyah Gamping pada periode Agustus sampai dengan September 2019. Berdasarkan hasil Uji validitas terhadap 15 item pertanyaan diperoleh 2 item pertanyaan yang tidak valid, selanjutnya item pertanyaan dihapus dan dilakukan validasi ulang dan diperoleh hasil valid dengan nilai koefisien pearson correlation ditasa nilai r tabel. Hasil uji reliabilitas menggunakan cronbach alpa diperoleh semua item pertanyaan memiliki nilai cronbach alpa diatas 0,6 sehingga dinyatakan reliabel. Kata Kunci: Validasi, pengetahuan, ibu hamil, suplemen zat besi     ABSTRACT Anemia in pregnancy is one of the problems in pregnancy. WHO data in 2001 showed that more than 50% of pregnant women had anemia. Anemia in pregnancy is a condition where the hemoglobin level is less than 11 mg / dL. The prevalence of anemia in Indonesia is quite high at 50-63%, while the prevalence in DI Yogyakarta is 14.32%. Research on the knowledge of pregnant women regarding their pregnancy conditions in Selangor shows that the knowledge of pregnant women is in the good category, but most of the subjects have low knowledge regarding iron supplementation. Research instruments on validated knowledge and supplementation of iron in Indonesia still do not exist, so data on knowledge of pregnant women regarding pregnancy and iron supplements is still lacking. The method in this study was to develop a research instrument (questionnaire) which was validated by three experts in their respective fields (content validation). The next step is to conduct a pilot test to find out input and feedback from subjects related to errors and confusing question items. Pilot tests are also used to measure the time to fill out the questionnaire. The final step is to construct validation conducted on 49 research subjects. Statistical analysis using Pearson correlation and Cronbach alpha was used to measure the validity and reliability of the questionnaire. Research subjects in this validation were 49 pregnant patients who received iron supplements and / or folic acid at PKU Muhammadiyah Gamping Hospital in the period of August to September 2019. Based on the results of the validity test of 15 question items, there were 2 items that were invalid, further items questions were deleted and re-validated and valid results obtained with the Pearson correlation coefficient ditasa r table values.
