Abstract

Immunization is a process of forming immunity in individuals against infectious diseases. The government has required infants to be fully immunized, but in practice, it turned out that there were still many people refusing immunization. This study aimed to analyze the coverage and the punctuality of immunization and the factors that influenced it. The research was descriptive observational with qualitative and quantitative approaches. The population of the study was mothers who had less than 2 years old infants living in Temanggung Regency. 498 and 199 respondents were obtained in 2018 and 2019 by using a random sampling technique. The research instrument used was Rapid Card Check and MCH Handbook. The results showed that infants were not immunized (2-6,5%), and the punctuality of immunization was 32-89%. There was a relationship between knowledge (p=0,039) and the mother's perception (p=0,005) towards infants' essential immunization completeness. Abstract

The trial implementation of the PrEP program as an HIV prevention program in Semarang City has not run optimally. Health workers with an essential role as program implementers still experience technical and technical obstacles in recording and reporting, which are quite complicated. It is necessary for a program that has been running for almost 3 years to carry out more in-depth studies, especially for health workers who play a direct role in providing PrEP services. Data collection was descriptive and qualitative, using in-depth interviews. The study subjects consisted of nine primary informants and four triangulated informants using purposive sampling. Data validity is carried out by triangulating data sources. Data reliability is measured by data audit and data analysis using the content analysis method. The study results show that health workers' implementation of the PrEP program differs from the management guidelines published by the Ministry of Health of the Republic of Indonesia in 2023. Most health workers have a low level of knowledge regarding the PrEP program and perceive that the PrEP program supports LGBT and free sex. As a result, intrapersonal conflict arises because it is considered to conflict with one's thought patterns and conscience. The conclusion is that the low level of knowledge among the majority of PrEP program implementers significantly influences the PrEP program's success. Therefore, there is a need for regular, more comprehensive training related to the PrEP program.

Abstract

Background: Coronary heart disease (CHD) ranks seventh among non-communicable diseases (NCDs) in Indonesia, with a projected increase in mortality by 2030 to approximately 23,6 million. The rehabilitation of CHD, which constitutes long-term treatment, significantly depends on patient adherence. Objective: This study aims to investigate the determinants of patient adherence behavior at the Bandung Heart Clinic. Methods: It employed a crosssectional research design, gathering data from 471 CHD patients with a history of atherosclerosis at the clinic between January and October 2022. The minimum sample size of 80 respondents, calculated using the Lemeshow formula, was increased to 150 to mitigate potential dropouts. The research instrument used was a questionnaire that had been validated and deemed reliable, except for one question regarding family support. Sampling was conducted using accidental sampling and involved interview data collection. Results: Bivariate data analysis, using the chi-square test, indicated significant associations between gender (p=0.09), employment status (p=0.01), health insurance participation (p=0.012), healthcare accessibility (p=0.022), family support (p=0.006), and treatment motivation (p=0.22) with patient adherence behavior. Additionally, female patients (OR=3,316) patients who are members of a health insurance (OR=2,617), patients with high treatment motivation (OR=1,983), patients who receive support from their families (OR=2,476) are more likely to exhibit adherence behavior compared to those who are not. Conclusion: In summary, it can be concluded that gender, employment status, health insurance participation, family support, and treatment motivation have significant correlations with patient adherence behavior in the context of CHD at the Bandung Heart Clinic.

Abstract

This article discusses the legal dynamics of the dual position of the Mayor of Batam as the Head of BP Batam. The focus of this research emphasizes the implications arising from the policy of Government Regulation No. 62/2019, which requires one individual to hold two strategic positions at once, namely as Mayor of Batam, who is authorized to manage the autonomous region and as Head of BP Batam who is authorized to manage investment and development of the Batam area into a special economic zone. This study aims to evaluate the implementation of this dual position policy. Using empirical legal research methods through interviews, literature review, and qualitative analysis, the findings indicate benefits such as increased synergy between economic development and public services. It allows for better coordination in managing the interests of the city and the industrial estate. However, this dual management structure risks weakening Batam's regional autonomy status and creating conflicts of interest that may affect the objectivity of decision-making. On the one hand, this policy is considered a practical solution to resolve the leadership dualism that has been occurring. On the other hand, this policy creates consequences and challenges. This study concludes that to optimize the benefits and minimize conflicts, it is necessary to harmonize more transparent regulations, more effective supervision, and the commitment of officials to prioritize the public interest for implementing local Government in Batam City.

Abstract

Central Java Province is one of the provinces that often experiences disasters. During the period 1 May 2023 – 31 May 2023, 72 incidents of strong winds, 8 incidents of land moving, 107 incidents of flooding, 94 incidents of landslides, 6 volcanic eruptions, and 2 incidents of fire were recorded. In disaster emergencies, the health needs of mothers and children are often overlooked. This study aims to analyze the flow of maternal and child health referrals in health crisis management in the focus location regencies in Central Java Province. This research is descriptive research with a qualitative approach. Data was collected through Focus Group Discussion (FGD) in March 2023. The research locations were Semarang Regency, Brebes Regency, Grobogan Regency, and Klaten Regency. The sample selection technique used was purposive sampling. The integrated referral system is not yet running optimally. The health referral flow carried out is not yet specialized in treating mothers and children. Pentahelix collaboration is carried out in managing health crises. The supporting factors in disaster management are the existence of policies, the availability of infrastructure for evacuating victims, as well as coordination from various sectors. Meanwhile, the inhibiting factors are the lack of trained human resources, limited logistics, and low public awareness regarding disasters. Thus, there is a need to develop a special referral pathway for maternal and child health involving multi-sectors, increase public awareness of disaster management, and strengthen Pentahelix collaboration in health crisis management.
