Abstract
Description
The invention relates to a dental implant for use in a jawbone.
It is known to use implants for the construction of crowns in the jaw bones, which are anchored in the jaw and have a secondary part which protrudes from the jaw and is used for the actual attachment of the dentures or the crown to be built thereon. The implant therefore serves as an artificial tooth root. Known implants are designed in the manner of a screw with a self-tapping thread, the primary part being screwed into the jaw and then the secondary part being put on. Also known are implants with a thread that are screwed into a predrilled hole. Such implants are usually made of titanium, since bone cells are later deposited and attached to this material and such implants grow particularly well with the jawbone. One problem, however, is that the implant can slide through the drilled hole into the maxillary sinus and have to be removed very laboriously or even surgically. Also the mechanical one The strength of an implant used in this way is not always optimal.
The invention is therefore based on the object of providing a dental implant for use in a jawbone which can be anchored particularly firmly in a jawbone and is also easy to handle.
This object is achieved with an implant with the features of patent claim 1. Advantageous developments of the invention are specified in the subclaims.
According to the basic idea of the invention, the implant is designed as an expandable sleeve over at least part of its length. With such an implant it is achieved that it is jammed in the jawbone and, as a result, the force transmission takes place substantially more uniformly and in a larger area than by screwing in the implant.
Insbesondere ist also kein zusätzliches Außengewinde vorhanden. The implant is preferably designed such that the sleeve has an inner space surrounded by a wall. The wall is advantageously designed so that it has at least one slot, so that an expandable sleeve is formed in this way. Other possibilities are also conceivable, for example by using a comparatively elastic material or a wall which is formed by material thickness and material in such a way that it can be stretched or expanded. A particularly reliable spreading of the implant is possible, however, by using a sleeve with a slot. Such slots can be formed in all directions. However, it is preferred to have the slot or slots parallel to the longitudinal axis of the sleeve train. The sleeve is preferably free on the outer surface of structures running essentially perpendicular to the longitudinal axis of the sleeve. In particular, there is no additional external thread. However, it is expedient to form the surface roughened, so that a frictional action and a frictional engagement already occur when the implant is inserted. It has also been shown that a later growth of the implant in the jaw is particularly durable with a roughened surface.
In a preferred development of the invention, the sleeve has an anti-rotation device. This can be formed, for example, by structures running on the outside of the sleeve parallel to the longitudinal axis of the sleeve. In particular, the sleeve has at least one web running parallel to the longitudinal axis on its outside. With a web of this type, when the implant is inserted into a predetermined bore, the implant is firmly seated in place. Subsequent expansion of the implant then firmly anchors it through the clamping action. The sleeve is particularly preferably designed such that, in an alternating arrangement, slots and webs running parallel to one another, in particular parallel to the longitudinal axis, are provided. In the preferred embodiment, a total of three slots and three webs are provided.
In a further preferred embodiment, the implant is wedge-shaped, so that the diameter of the lower end, which is first inserted into the bore, is smaller than the diameter of the upper region. This reliably prevents the implant from being pushed through and lost. Alternatively, it is also possible that the area of the sleeve of the implant has a constant diameter and a head with a larger diameter, in particular with a conically widening diameter is arranged above the sleeve, which reliably prevents pushing through. The implant is preferably made of titanium.
In a preferred development of the invention, the implant has an associated locking and expansion screw which can be screwed into the interior of the sleeve. For this purpose, an internal thread is preferably arranged in the interior of the sleeve, into which the secondary part can be screwed. The locking and expanding screw preferably has a head which closes the interior of the sleeve at its open end, wherein a recess for an internal hexagon is preferably provided in this head, so that the locking and expanding screw can be screwed into the implant.
Furthermore, the implant is preferably also assigned a secondary part, which likewise has a head which closes the opening of the interior of the implant at its open end and has a device for building up a crown. This secondary part can also be screwed into the interior of the sleeve into the internal thread there. The dimensions of the locking and expansion screw and also the secondary part, in particular the diameter of the area that can be screwed into the interior of the implant, are larger than the interior of the implant, so that when the locking and expanding screw or the secondary part is screwed in, the implant, in particular the area the sleeve of the implant is expanded or spread out with the interior and is thereby firmly and evenly jammed in the surrounding jawbone. The secondary part will Conveniently used after a transition period of a few days or weeks, in which the locking and expanding screw was screwed into the implant, instead of this locking and expanding screw. In some cases, the secondary part can also be screwed directly into the directly inserted implant. The areas of the secondary part which can be screwed into the sleeve and its dimensions are preferably somewhat smaller than the corresponding parts of the locking screw and expansion screw. The locking and expanding screw and the secondary part and its external thread and the corresponding internal thread in the sleeve of the implant are designed so that after inserting the implant into the jawbone and screwing in the locking and expanding screw or the secondary part, the sleeve and also in slots provided in the sleeve are completely sealed so that bacteria or granulation tissue cannot penetrate.
This patent also claims the locking and expansion screw and the secondary part for use with the above-mentioned implant.
The invention is explained in more detail below on the basis of a preferred exemplary embodiment shown in the drawing.
Im einzelnen zeigen die schematischen Darstellungen in :In detail, the schematic representations show in:
Fig. 1 eine Querschnittsansicht eines erfindungsgegemäßen Implantats; Fig. 2 einen Schnitt durch ein Implantat gemäß Fig. 1 entlang der Linie II-II; Fig. 3 eine Querschnittsansicht einer Verschluß- und Aufdehnschraube; und Fig. 4 eine Querschnittsansicht eines Sekundärteils .1 shows a cross-sectional view of an implant according to the invention; FIG. 2 shows a section through an implant according to FIG. 1 along the line II-II; 3 shows a cross-sectional view of a locking and expansion screw; and Fig. 4 is a cross-sectional view of a secondary part.
Der Kopf 3 weist zudem einen Innenraum 9 auf, der in seinem Durchmesser größer ist als der Innenraum 5.1 shows a section through an implant 1, which over part of its length is designed as a sleeve 2, which can be expanded, and in a further region as a head 3. The implant 1 has an interior 5 and a wall 4 surrounding this interior 5. The implant 1 is essentially cylindrical, but has webs 6 running on its outside parallel to the longitudinal axis of the implant 1. The webs 6 extend essentially over the area that is also occupied by the interior 5 in the implant. The front end region 7 of the implant 1 is hemispherically rounded and then merges into the cylindrical shape of the rest of the implant 1. 1, a slot 8 can be seen in the upper region, which runs parallel to the longitudinal axis of the implant and also extends essentially over the entire length of the sleeve 2, in particular essentially over the length of the interior 5 of the implant 1, extends and thereby allows the implant 1 to be spread apart. The interior 5 is designed to taper towards the front end region 7 and a thread is arranged on the wall side of the wall 4 facing the interior 5. The head 3 of the implant 1 is of a larger diameter than the sleeve 2, so that the implant is reliably prevented from being pushed through an opening drilled in the jawbone into the maxillary sinus behind it. The head 3 also has an interior 9 which is larger in diameter than the interior 5.
In cross section is the cylindrical shape of the implant 1 and the sleeve 2 clearly visible. A total of three slots 8 are provided in the wall 4, which extend through the entire wall 4, that is to say from the interior 5 to the outside, and thereby enable the sleeve 2 to be spread apart. The front end region 7 and also an upper region are not covered by the slots 8, so that the sleeve is retained in its overall shape. The slots 8 are arranged symmetrically to one another, ie in the present exemplary embodiment that in the case of three slots they are spaced apart from one another at an angle of 120 °. Furthermore, three webs 6 are provided here, which are also arranged symmetrically to one another at an angle of 120 ° and are each displaced to the slots 8 by an angle of 60 °. The webs 6 are tapered in cross-section with a front edge designed as a cutting edge, which act like a cutting edge when inserted into a predefined hole in the jawbone and prevent twisting and unintentional loosening or removal of the implant already in the treatment stage.
Such a closure and expansion screw, which has an external thread 14 in the area of the pin element 12, can be screwed into the corresponding internal thread in the interior 5 of the implant. The contours of the pin element 12 are adapted to the contours of the interior 5. The dimensions of the pin element 12 with the external thread 14 are, however, slightly larger than the dimensions of the interior space 5. The diameter of the pin 12 can be approximately 20% larger than the diameter of the interior space 5. This results in the screwing of the locking and expansion screw 10 causes the sleeve 2 to spread into the implant 1, so that the implant 1 is then pre-drilled

In addition, a recess 13 for an internal hexagon is arranged in the head 11, so that the locking and expanding screw 10 is mechanically simple to handle and can be screwed into the implant 1.
4 shows a cross-sectional view of a secondary part 20, which can also be inserted into the implant 1. The secondary part 20 can either be inserted directly into the implant 1 or only after the locking and expansion screw 10 has been screwed into the implant 1 for a transitional period. The secondary part 20 also has a pin element 22 with an external thread 24 which can be screwed into the interior 5. The dimensions of the pin element 22 of the secondary part 20 are, in particular, somewhat larger in circumference than those of the interior 5 of the implant 1. However, they are advantageously somewhat smaller than those of the pin element 22 of the locking and expanding screw 10. The diameter of the pin element 22 of the secondary part 20 is approximately 10% larger than that of the interior 5 of the implant 1. In the upper region, the secondary part 20 has a head 21 whose dimensions are adapted to the recess 9 of the head 3 of the implant 1 and completely closes it. Both by screwing in the secondary part 20 and by screwing the locking and expansion screw 10 into the implant 1, the latter expands. On the secondary part 20 there is also a build-up element 23 which projects beyond the actual implant 1 and on which the crown is then built up. In this structural element 23 is also provided with a recess for an internal hexagon.
