Abstract

La présente invention est un système complémentaire [A] d'une d'une osteosynthése vertébrale postérieure appuyée sur des vis pédiculaires [B]. Il vise à maintenir et fixer des greffes osseuses compactes [F], d'une arthrodèse postérolatérale contre la face postérieure des apophyses transverses (8). Il s'agit d'une lamelle (1) fixée par un moyen quelconque [C] sur sur les têtes (2) des vis pédiculaires [B], indépendamment du modèle de connecteur [D] avec avec les barres de solidarisation verticales [E] du montage.

DISPOSITIF de FIXATION des GREFFES OSSEUSES POSTERIEURES et LATERALES pour les ARTHRODESES LOMBAIRES et LOMBOSACREES en COMPLEMENT d'une OSTEOSYNTHESE avec VIS TRANSPEDICULAIRES.
La présente invention est un dispositif qui vise à fixer les greffes osseuses sur le versant posterieur et lateral de la colonne vertebrale : les massifs articulaires et la face posterieure des apophyses transverses.
Le dispositif est conjugué à une osteosynthése posterieure dont la caracteristique commune est de combiner un montage par des vis transpediculaires et une solidarisation de l'ensemble par des barres verticales (Fig. 1 et 2).
Le système inventé s'appuie sur les têtes de vis transpediculaires de l'osteosynthése, quelques soient le type de vis utilisé et le type de connection avec le dispositif de barres verticales de solidarisation.
Une fois l'osteosynthése en place, des greffes compactes sont placées dans les gouttières posterolaterales préalablement avivées, puis le dispositif de complement est fixé sur les têtes de vis. Il applique mecaniquement les greffes sur le lit osseux receveur : le versant lateral des massifs articulaires et la face posterieure des apophyses transverses, comme il est indiqué sur la figure 1 où l'on voit que sur le côté gauche est montré le montage de l'invention avec une greffe d'os compact, tandis que du côté droit, on a le schema traditionnel de la greffe d'os morcelé sans fixation complementaire.
Ainsi l'intérêt de cette invention est, desormais, de ne plus confier au hasard la fusion d'une arthrodése posterolaterale, en usant d'un moyen mecanique

supplementaire pour fournir une plus grande chance de reussite à l'operation : Le système [A] est composé avant tout d'une lame (1) (Fig. 3 et 4) dont la base presente un moyen de connection qui permet au dispositif d'être appliqué sur la tête (2) des vis pediculaires [B]. Ce moyen de connection peut varier : ce peut être comme sur la figure 3, le modèle simple composé d'un anneau (3) et d'un orifice (4) qui visent à menager le passage de la vis de fixation [C] qui ici, est introduite dans le logement de la tête (2) de la vis pediculaire [B]. Mais ce mode de fixation par une vis [C] introduite dans la tête de vis pediculaire (2) peut être d'un tout autre type selon le modèle de vis [B] utilisé et du connecteur [D], qui solidarise les vis pediculaires [B] aux barres verticales [E], lesquelles assurent la solidarisation entre les vertebres. Ainsi l'invention doit pouvoir être adaptée à tout type d'osteosynthése present ou à venir basé sur le principe de l'ancrage par vis transpediculaires independamment du mode de liaison avec les barres verticales de solidarisation intervertebral.
Un autre de modèle de pièce [A] est prevu où la lame (1) est en continuité et perpendiculaire à une plaque (5), de longueur variable (fig. 4), pour prevoir un montage en hauteur sur plusieurs vertébres. Cette plaque est ajourée par une fenêtre centrale (6) longitudinale, qui permet ainsi le passage des vis de fixation [C] sur au moins 2 etages vertebraux comme sur la figure (1), afin d'eviter

quelques difficultés du fait des variations imprevisibles d'ecart en hauteur des points d'entrée des vis [B] dans les pedicules vertebraux.
Le dispositif [A] presente en outre à son extrémité 2 ergots (7) qui, combi- nés à l'incurvation et à la souplesse de la pièce, visent à mieux maintenir la greffe compacte [F] et ainsi à mieux l'appliquer contre la face posterieure des apophyses transverses (8) de la vertebre.

Claims (3)

REVENDICATIONS
1. Système de fixation [A] des greffes osseuses compactes posterolaterales adjointes à une osteosynthése vertebrale posterieure par vis transpediculaires [B] pour ameliorer les chances de fusion d'une arthrodése intervertebrale.
Le système [A] est caractérisé par le fait d'être une lamelle (1) munie à une 'de ses extrémités d'un anneau (3) qui menage un orifice (4) pour le passage d'un moyen de fixation telle une vis [C] elle même fixée sur la tête de vis (2) des vis pediculaires [B] qui assurent l'ancrage osseux de l'osteosynthése indépendamment du mode de connection [D] avec les barres de solidarisation [E] du montage.
2. Dispositif selon la revendication 1, caracterisé en ce que la lame (1) de la pièce [A] est munie à son autre extremité de 2 ergots qui combinés à l'incurvation et à la souplesse du dispositif permettent de bien appliquer la greffe d'os compact [F] contre les apophyses transverses (8) de la vertebre instrumentée.

Dispositif selon les revendications 1 et 2, caractérisé en ce que la pièce [A] est une variante du modèle simple par le fait que la lame (1) est en continuité à 90 avec une plaque (5) ajourée par une fenêtre centrale longitudinale (6) qui permet de fixer la pièce [A] sur 2 ou plusieurs etages vertebraux sans être entravé par une variation imprevisible d'ecart entre les têtes (2) des vis [B].
