Disclosure of Invention
The invention aims to provide a medical follow-up visit system, so that the follow-up visit of patients is not limited in hospitals, the follow-up visit management is accurately and timely carried out, and the time of doctors and patients is greatly saved.
In order to solve the above technical problem, the present invention provides a medical follow-up system, including: a doctor end, a patient end and a server end; wherein,
the doctor end is used for establishing association with a patient end corresponding to a follow-up object, sending a follow-up prescription and a medicine scheme to be filled to the patient end through the server end, and acquiring the filled follow-up prescription and medicine information submitted by the patient end;
the patient end is used for receiving the follow-up prescription sent by the doctor end through the server end, filling in the follow-up prescription, checking the medicine taking scheme sent by the doctor end and filling in the medicine taking information for the doctor end to check;
the server end is used for processing and transmitting data between the doctor end and the patient end.
Optionally, in the medical follow-up system, the doctor end includes a hospital data management module, and the hospital data management module is configured to create and view patient information of a follow-up subject, where the patient information includes an examination report of an examination performed in a hospital.
Optionally, in the medical follow-up system, the doctor end includes a follow-up prescription management module, and the follow-up prescription management module is configured to create a follow-up prescription to be filled on a preset prescription suggestion frame according to the patient information of a follow-up subject, push the follow-up prescription to be filled to the patient end through the server for filling by the follow-up subject, and obtain the filled follow-up prescription submitted by the patient end and/or an analysis result of the filled follow-up prescription by the server through the server.
Optionally, in the medical follow-up system, the follow-up prescription includes a plurality of follow-up items, and a follow-up period and a follow-up frequency corresponding to each of the follow-up items.
Optionally, in the medical follow-up system, the doctor end includes a follow-up medication management module, and the follow-up medication management module is configured to send the medication scheme to the patient end through the server end, check the medication information filled in by the patient end, update the medication scheme according to the medication information filled in by the patient end, and send the medication scheme to the patient end.
Optionally, in the medical follow-up system, the medical follow-up system further includes a sensor end, and the sensor end is configured to be disposed on the home inspection device, so as to send inspection data of the home inspection device to the server end; the doctor end comprises an extramural data viewing module, and the extramural data viewing module is used for viewing the inspection data uploaded to the server end by the sensor end and/or viewing a report generated by processing the inspection data by the server end.
Optionally, in the medical follow-up visit system, the doctor end includes a doctor-patient binding module, and the doctor-patient binding module is configured to establish an association between the doctor end and the patient end.
Optionally, in the medical follow-up system, the patient end includes a follow-up prescription filling module, and the follow-up prescription filling module is configured to fill the follow-up prescription to be filled and sent by the doctor end through the server end, submit the filled follow-up prescription to the server end, and send the filled follow-up prescription to the doctor end through the server end.
Optionally, in the medical follow-up visit system, the patient side includes a follow-up visit medicine filling module, and the follow-up visit medicine filling module is configured to check the medicine taking scheme of the medicine to be taken sent by the doctor side and fill the medicine taking information for the doctor side to check.
Optionally, in the medical follow-up visit system, the follow-up visit medicine filling module has a regular reminding function, and is used for reminding a follow-up visit subject to fill in the medicine information and feed back the medicine information to the doctor end; or the follow-up visit medicine filling module has an active triggering function, and after the patient end receives the medicine taking scheme sent by the doctor end, the follow-up visit medicine filling module actively triggers a filling instruction of the medicine taking information to remind a follow-up subject to fill the medicine taking information and feed the medicine taking information back to the doctor end.
Optionally, in the medical follow-up visit system, the doctor end includes an appointment reminding module, and the appointment reminding module is used for the doctor to initiate a next outpatient service appointment reminding operation for a follow-up visit object and send the follow-up visit object to the patient end through the server end; the patient end comprises a message receiving module, and the message receiving module is used for receiving follow-up prescription filling reminding information, medication information filling reminding information and outpatient service appointment reminding information which are pushed by the server end and come from the doctor end.
Optionally, in the medical follow-up system, the server is further configured to analyze the follow-up prescription filled by the patient end to determine whether data on the follow-up prescription is out of a normal value range, and if so, send an abnormal early warning signal to the doctor end and/or the patient end.
In summary, in the medical follow-up system provided by the present invention, the medical follow-up system includes: a doctor end, a patient end and a server end; the doctor end is used for establishing association with a patient end corresponding to a follow-up object, sending a follow-up prescription and a medicine scheme to be filled to the patient end through the server end, and acquiring the filled follow-up prescription and medicine information submitted by the patient end; the patient end is used for receiving the follow-up prescription sent by the doctor end through the server end, filling in the follow-up prescription, checking the medicine taking scheme sent by the doctor end and filling in the medicine taking information for the doctor end to check; the server end is used for processing and transmitting data between the doctor end and the patient end.
Compared with the prior art, the medical follow-up system provided by the invention has the following disturbance:
(1) the doctor carries out information creation, follow-up prescription sending and medicine taking scheme sending through the doctor end, and the patient fills in the follow-up prescription through the patient end, so that the patient can complete follow-up on line without going to a hospital, the follow-up efficiency can be effectively improved, the follow-up difficulty is reduced, and the application scene of follow-up is enlarged;
(2) the patient uploads the medicine taking condition in real time through the patient end, and the doctor can check the medicine taking information of the patient through the doctor end, so that the doctor can more comprehensively know whether the patient has other medicine taking conditions besides the medicine taking scheme set by the patient at present, and the set medicine taking scheme can be adjusted more timely;
(3) furthermore, after the patient uses the household monitoring equipment to perform self-test and upload the self-test data to the server, the server processes the examination data to generate a diagnosis report, so that a doctor can know the out-of-hospital body state of the patient regularly or irregularly through the doctor, and sets a medicine taking scheme according to the current state of the patient, thereby effectively improving the accuracy of follow-up visits.

To make the objects, advantages and features of the present invention more apparent, the present invention will be described in detail below with reference to the accompanying drawings and specific embodiments. It is to be noted that the drawings are in greatly simplified form and are not to scale, but are merely intended to facilitate and clarify the explanation of the embodiments of the present invention. As shown in fig. 1, the present embodiment provides a medical follow-up system including: a doctor end 1, a patient end 2 and a server end 3, wherein,
the doctor end 1 is used for establishing association with a patient end 2 corresponding to a follow-up visit object, sending a follow-up visit prescription and a medicine scheme to the patient end 2 through the server end 3, and acquiring the filled follow-up visit prescription submitted by the patient end 2 and checking medicine information filled by the patient end 2;
the patient end 2 is used for receiving the follow-up prescription sent by the doctor end 1 through the server end 3, filling in the follow-up prescription, checking the medicine taking scheme sent by the doctor end 1 and filling in the medicine taking information for the doctor end 1 to check;
the server 3 is used for processing and transmitting data between the doctor end 1 and the patient end 2.
According to the medical follow-up system provided by the embodiment, a doctor sends a follow-up prescription and a medicine taking scheme through the doctor end 1, and a patient fills the follow-up prescription and the medicine taking condition through the patient end 2, so that the patient can complete follow-up on line without going to a hospital, the follow-up efficiency can be effectively improved, the follow-up difficulty is reduced, and the application scene of follow-up is enlarged; in addition, the patient uploads the medicine taking situation in real time through the patient end 2, and the doctor can check the medicine taking information of the patient through the doctor end 1, so that the doctor can know more comprehensively whether other medicine taking situations exist besides the current medicine taking scheme set by the patient, and the medicine taking scheme set by the doctor can be adjusted more timely.
In the medical follow-up system provided by this embodiment, the doctor end 1, the patient end 2, and the server end 3 may be connected via a wide area internet.
The doctor end 1, the patient end 2 and the server end 3 are described in further detail below.
In this embodiment, the doctor end 1 is mainly used by a doctor, and a hardware carrier thereof is, for example, a smart phone or a computer. The doctor end 1 may specifically include an in-hospital data management module 11, a follow-up prescription management module 12, a follow-up medication management module 13, an out-of-hospital data viewing module 14, and a doctor-patient binding module 15.
The hospital data management module 11 is configured to create and view patient information of a follow-up object, where the patient information includes examination reports of examinations performed in a hospital, and may specifically include examination items and corresponding data.
The follow-up prescription management module 12 is configured to create a follow-up prescription on a preset prescription suggestion frame according to the patient information of a follow-up subject, push the follow-up prescription to be filled to the patient terminal 2 through the server terminal 3 for filling by the follow-up subject, and obtain the filled follow-up prescription submitted by the patient terminal 2 and an analysis result of the filled follow-up prescription of the patient terminal 2 by the server terminal 3 through the server terminal 3.
Of course, in other embodiments, the filled follow-up prescription may also be directly sent to the follow-up prescription management module 12 through the server 3 for the doctor to review, and the server 3 does not perform analysis on the follow-up prescription, or the server 3 may also directly send the analysis result of the follow-up prescription filled by the patient end 2 to the follow-up prescription management module 12 for the doctor to review, which is not limited in the present invention.
The follow-up prescription can comprise a plurality of follow-up items, and a follow-up period and a follow-up frequency corresponding to each follow-up item. Through the follow-up prescription management module 12, a doctor can know the physical condition of a follow-up subject in real time, so that a proper medicine is selected for the follow-up subject, and the accuracy of follow-up visit can be improved.
In practical application, a doctor can customize and customize an out-of-hospital follow-up item, a follow-up period and a follow-up frequency for a follow-up object according to the actual condition of the follow-up object, and the follow-up item is pushed to the patient end 2 through the server end 3 for the follow-up object to fill in, so that the doctor can check the follow-up prescription filled in by the patient through the follow-up prescription management module 12 and the result of preliminary analysis of the follow-up prescription filled in by the patient through the server end 3.
For example, the follow-up prescription set by the doctor includes follow-up information-symptoms, EHRA (european heart rhythm association) score, quality of life score, physical examination-blood pressure, blood sugar, examination-electrocardiogram, and the follow-up subject can be filled in accordingly.
The follow-up medication management module 13 is configured to send a medication scheme, such as a medication name, a medication duration, and when to take medication, to the patient end 2 through the server end 3, check the medication information filled in by the patient end 2, update the medication scheme according to the medication information filled in by the patient end 2, and send the medication scheme to the patient end 2.
Preferably, the medical follow-up system provided by this embodiment further includes a sensor terminal 4, where the sensor terminal 4 is configured to be disposed in a household examination device (such as a household electrocardiographic device, a sphygmomanometer, an oximeter, etc.) to send examination data of the household examination device to the server terminal 3; the extramural data viewing module 14 is configured to view the inspection data uploaded to the server 3 by the sensor terminal 4 and/or view a report generated by processing the inspection data by the server 3.
The doctor-patient binding module 15 is used for establishing the association between the doctor end 1 and the patient end 2. For example, the doctor-patient binding module 15 is a two-dimensional code generation module for generating a two-dimensional code dedicated to a specific doctor, the patient can scan the doctor-specific two-dimensional code provided by the doctor end 1, enter the patient end 2, fill in relevant information, apply for joining and bind with the doctor end 1 to establish a connection, and the doctor can realize the association between the doctor end 1 and the patient end 2 only after the doctor end 1 approves the application of the patient.
In addition, the doctor end 1 may further include an appointment reminding module 16, and the appointment reminding module 16 is configured to enable a doctor to initiate a next outpatient appointment reminding operation for a follow-up object, and send the follow-up object to the patient end 2 through the server end 3. When the time of outpatient service is up, or when the doctor considers that the patient needs to go further to the hospital for detailed examination through the follow-up prescription filled in by the follow-up object, the appointment reminding module 16 can send out the outpatient service appointment reminding information to the patient end 2 to remind the follow-up object of outpatient service in time.
Patient end 2 is used for the follow-up object to use, and in this embodiment, patient end 2 can be based on believe APP realizes a little, and its hardware carrier equally can be smart mobile phone or computer etc.. The patient end 2 may specifically include a follow-up prescription filling module 21 and a follow-up medication filling module 22.
The follow-up prescription filling module 21 is configured to allow a follow-up subject to fill the follow-up prescription sent by the doctor end 1 through the server end 3, submit the filled follow-up prescription to the server end 3, and send the follow-up prescription to the doctor end 1 through the server end 3.
Specifically, the follow-up prescription filling module 21 is associated with the follow-up prescription management module 12 of the doctor end 1, fills the follow-up prescription after receiving the follow-up prescription to be filled, which is sent by the doctor through the follow-up prescription management module 12, and then sends the follow-up prescription to the follow-up prescription management module 12 through the server end 3.
The follow-up visit medicine filling module 22 is configured to check a medicine taking scheme of the medicine to be taken sent by the doctor end 1 and fill in medicine taking information for the doctor end 1 to check, where the medicine taking information includes a medicine taking condition required in the medicine taking scheme and a medicine taking condition of the follow-up visit subject taking other required medicines.
Specifically, the follow-up visit medicine filling module 22 is associated with the follow-up visit medicine management module 13 of the doctor terminal 1, so that a doctor can comprehensively know the medicine taking condition of a patient in real time, and the doctor can better guide the patient to take medicine. In addition, the follow-up medication reporting module 22 also provides the patient with a platform to update the physician in real time with the medication that he is taking. Specifically, the follow-up subject may have other old diseases or newly-added other diseases besides the medicines of the follow-up diseases, for example, the follow-up subject may have improper medication or even adverse consequences of medicine conflict because the follow-up subject cannot communicate with the doctor in the same way or forgets to communicate with the doctor and does not inform the doctor of the information in time during the follow-up process. In this embodiment, the follow-up subject can fill in the medication information in the follow-up medication filling module 22, so that the doctor can know the diseases of the patient more comprehensively, thereby avoiding the burden that the patient needs to go to a hospital for a doctor to see a doctor once the medication condition of the patient changes, for example, the medication of other diseases is newly added, or the risk of improper medication caused by failure of timely informing the doctor is avoided.
Further, in order to prevent the patient from forgetting to fill in the information of the medicine being taken, the follow-up medicine filling module 22 in this embodiment may have a regular reminding function to remind the follow-up staff to fill in the information of the medicine and feed back the information to the follow-up medicine management module 13; alternatively, the follow-up visit medicine filling module 22 may also have an active triggering function, and after the patient end 2 receives the medicine taking scheme sent by the follow-up visit medicine management module 13, the follow-up visit medicine filling module 22 actively triggers a medicine taking filling instruction to remind a follow-up person to fill in medicine information and feed back the medicine information to the follow-up visit medicine management module 13, which is not limited in this application.
As described above, in this embodiment, the doctor-patient end 1 includes the doctor-patient binding module 15, and the doctor-patient binding module 15 is configured to establish an association between the doctor end 1 and the patient end 2 at the doctor end 1. In other embodiments, the association between the patient end 2 and the doctor end 1 can be established at the patient end 2. For example, the relevant information may be filled in on the login page of the patient end 2, a specific hospital and a doctor are selected, an application is initiated to join and be associated with the specific doctor, and after the doctor end 1 approves the above application of the patient, the association between the patient end 2 and the doctor end 1 is realized, which is not limited in the present application.
In addition, the patient end 2 may further include a message receiving module 23, and the message receiving module 23 is configured to receive the follow-up prescription filling reminding information, the medication information filling reminding information, the outpatient service appointment reminding information, and other messages from the doctor end 1, which are pushed by the server end 3. When receiving the follow-up prescription filling reminding information, the patient can fill the follow-up prescription through the follow-up prescription filling module 21, when receiving the medication reminding information, the patient can feed back the information of taking the medicine to the server 3 through the follow-up medicine filling module 22, the server 3 records the medicine taking condition of the patient, and the medicine taking condition is synchronized to the follow-up medicine management module 13 for the doctor to check. The message pushing mode is various, for example: by paying attention to the WeChat service number or by pushing through a mailbox, etc.
The doctor end 1 and the patient end 2 safely, stably and quickly realize data transmission through the server end 3. In addition, the server 3 also has functions of early warning, reminding, message pushing and the like, and has the following aspects:
the server 3 can push the follow-up prescription customized by the doctor to the follow-up prescription filling module 21 in the patient end 2;
the server 3 sends the medicine taking reminding message to a message receiving module 23 in the patient end 2;
the server 3 records the medicine taking condition of the patient and feeds the medicine taking condition back to a follow-up medicine management module 13 in the doctor follow-up module for the doctor to check;
the server 3 sends the outpatient service appointment reminding message set by the doctor to a message receiving module 23 in the patient end 2;
the server 3 records and processes the examination data uploaded by the monitoring devices (such as household electrocardio devices, sphygmomanometers, oximeters and the like) used by the patient, and generates a real-time dynamic report (such as an electrocardio report or a Holter report).
In addition, the server 3 is further configured to analyze the follow-up prescription filled by the patient end 2 to determine whether data on the follow-up prescription exceeds a normal value range, and if so, send an abnormal early warning signal to the doctor end 1.
The following is a workflow of the medical follow-up system exemplified by the embodiment, including the following steps:
(1a) establishing an association with the patient side 2
The doctor scans the patient with the doctor-patient exclusive two-dimensional code of the doctor-patient binding module 15, the patient enters the patient end 2, after relevant information is filled in, the doctor applies for binding association with the doctor, and after the doctor approves the association application of the patient, the doctor establishes association with the patient end 2.
(1b) Doctor sets follow-up prescription
After the doctor end 1 and the patient end 2 are bound and associated, the doctor sets the follow-up prescription through the follow-up prescription management module 12 according to the actual situation of the patient.
(1c) Push follow-up prescription to be filled
After the doctor finishes setting the follow-up prescription, the follow-up prescription is submitted to the server 3, and the follow-up prescription to be filled is pushed to the follow-up prescription filling module 21 of the patient end 2 through the server 3.
(1d) Follow-up prescription for checking completion of filling of patient
After the doctor end 1 receives the filled follow-up prescription uploaded by the patient end 2, the doctor checks the filled follow-up prescription through the follow-up prescription management module 12 and/or the analysis result of the server end 3 on the follow-up prescription.
(1e) Checking the medicine taking condition filled by the patient and making or updating the medicine taking scheme
After receiving the medication conditions uploaded by the patient end 2, including the information such as the name and the dosage of the medication, the doctor end 1 formulates or modifies the medication scheme and sends the formulated or updated medication scheme to the message receiving module 23 through the follow-up medication management module 13.
The following is another workflow of the medical follow-up system exemplified by the present embodiment, including the following steps:
(2a) establishing association with the doctor terminal 1
The doctor scans the patient with the doctor-patient binding module 15 dedicated two-dimensional code, the patient enters the patient end 2, after filling in related information, the doctor applies for binding with the patient, and after the doctor approves the patient's associated application, the association between the patient end 2 and the doctor end 1 is established.
(2b) Collecting and recording patient self-test data
The patient self-detects the detection items in the follow-up prescription sent by the doctor at home, and fills related data in a follow-up prescription filling module 22 of the patient;
(2c) sending self-test data
The data measured by the patient can be filled in the follow-up prescription on one hand, and can be directly uploaded to the server terminal 3 on the other hand, and a report is generated by the server terminal 3 and pushed to the out-of-hospital data viewing module 14 of the doctor terminal 1.
In summary, the medical follow-up system provided by the present invention includes: a doctor end, a patient end and a server end; the doctor end is used for establishing association with a patient end corresponding to a follow-up object, sending a follow-up prescription and a medicine scheme to be filled to the patient end through the server end, and acquiring the filled follow-up prescription and medicine information submitted by the patient end; the patient end receives the follow-up prescription sent by the doctor end through the server end, fills in the follow-up prescription, checks the medicine taking scheme sent by the doctor end and fills in the medicine taking information for the doctor end to check; the server end is used for processing and transmitting data between the doctor end and the patient end. Therefore, the patient can complete follow-up visit on line without going to a hospital, so that the follow-up visit efficiency can be effectively improved, the follow-up visit difficulty is reduced, and the application scene of the follow-up visit is enlarged; in addition, the patient passes through the patient and holds the circumstances of taking medicine, and the doctor can hold through the doctor and look over the patient information of taking medicine to can make the doctor know more comprehensively that the patient is present except the scheme of taking medicine that it offered, whether other circumstances of taking medicine still have, thereby can adjust the medicine scheme of taking medicine of its setting more in time.
It should be understood, however, that the intention is not to limit the invention to the particular embodiments described. It will be apparent to those skilled in the art that many changes and modifications can be made, or equivalents employed, to the presently disclosed embodiments without departing from the intended scope of the invention. Therefore, any simple modification, equivalent change and modification made to the above embodiments according to the technical essence of the present invention will still fall within the protection scope of the technical solution of the present invention.
