Abstract

Outpatient treatment is one of the benefits guaranteed by the National Health Insurance (JKN) program. The main objective of JKN program is to increase public accessibility to health services that suits their needs. Utilization of outpatient services are getting better by all levels of society is an indicator to the ideal image of the JKN program. The purpose of this study was to determine the pattern of outpatient health care at health facilities (physician practices/clinics, public health center, public and private hospitals) in the first year implementation of JKN program. This study used secondary data to analyze the data of Indonesia Family Life Survey (IFLS) in 2014 as the first year of implementation JKN program. The data in this study using a descriptive analysis which was deeply reviewed.The results showed that in the span of one year of JKN program implementation, the age population who get access to outpatient health care at the most in the range 0-18 years, the people of the urban areas more access outpatient health services compared to rural residences, and the population in the range of quintile (rich economic status) as much as 26.73%. Health facilities are the most widely used for outpatient services is physician practice or clinic amounted to 4.55%. The results of this study can be used as an evidence-based evaluation of the improvement of health care access for the public to pay more attention to the economic status of poor and be a proactive improvement of access to health services for people in rural areas.   Abstrak Pelayanan rawat jalan merupakan salah satu manfaat yang dijamin oleh program Jaminan Kesehatan Nasional (JKN). Tujuan utama program JKN adalah meningkatkan aksesibiltas masyarakat terhadap pelayanan kesehatan yang sesuai dengan kebutuhan. Pemanfaatan layanan rawat jalan yang semakin baik oleh semua lapisan masyarakat merupakan gambaran ideal dan indikator tercapainya program JKN. Tujuan penelitian ini adalah untuk mengetahui pola pelayanan kesehatan rawat jalan pada fasilitas kesehatan (dokter praktik/klinik, puskesmas, RS pemerintah, dan swasta) di tahun pertama implementasi program JKN. Penelitian ini menggunakan data sekunder dengan menganalisis data Indonesia Family Life Survey (IFLS) 2014 sebagai tahun pertama implementasi program JKN. Teknik analisis dalam penelitian ini menggunakan analisis deskriptif yang diulas secara mendalam. Hasil penelitian menunjukkan bahwa pada rentang satu tahun implementasi program JKN, usia penduduk yang melakukan akses ke pelayanan kesehatan rawat jalan paling banyak pada rentang usia 0–18 tahun,masyarakat daerah perkotaan lebih banyak mengakses pelayanan kesehatan rawat jalan dibandingkan dengan penduduk pedesaan, dan penduduk pada kisaran kuintil 5 (status ekonomi kaya) sebanyak 26,73%. Fasilitas kesehatan yang paling banyak digunakan untuk layanan rawat jalan adalah dokter praktik/klinik sebesar 4,55%. Hasil penelitian ini dapat digunakan sebagai evidence based evaluasi perbaikan akses pelayanan kesehatan untuk lebih memperhatikan masyarakat berstatus ekonomi miskin dan jemput bola peningkatan akses pelayanan kesehatan bagi masyarakat di daerah pedesaan.

Abstract

Background: Indonesia’s experience in handling the Covid-19 pandemic is essential to prepare for future pandemic.Aim: This research aims to know the experience of health services in handling Covid-19 pandemic in nine provinces of Indonesia.Methods: This research used qualitative and case study approach, with a total of 94 informants coming from all levels of health service agencies from nine provinces in Indonesia. The data collection process was carried out by in-depth interviews, and data analysis were done by means of triangulation of sources between informants.Results: Each province has a variety of local policies with the addition of varying amouns of the budget for handling Covid-19. In addition, the workload of health workers increased after the pandemic, and the availability of facilities, medicines, and single-usage medical materials were deemed inadequate in dealing with the Covid-19 pandemic.Discussions: Accounting for the use of funds for handling Covid-19 as well as increasing surveillance capacity needed to prepare for future pandemic in Indonesia.

Abstract

Junk food consumption increases the risk of having symptoms of mental health problems. This study aimed to conduct a meta-analysis to assess the association between junk food and symptoms of mental health problems. Six researchers, two primary researchers, and four assistant researchers, from October to December 2020 conducted a systematic literature review. The data sources were selected from Pubmed and Science Direct articles published from 2010 to 2020. Those websites were check-marked for text availability for original articles, using keywords for junk foods and mental health. This study had inclusion criteria for selecting and organizing articles using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guideline. The full-text articles were selected for conducting a meta-analysis using R Studio Software. The 5,079 article titles were obtained, seven of which met the relevant requirements for meta-analysis. The range of respondents who experienced symptoms of mental illness was 1.38%–79.8%. There was no heterogeneity based on the Tau-square test. The correlation coefficient was 0.11 (95% CI 0.09–0.14), with no publication bias based on Egger’s Regression test (0.6023 or p-value>0.05). The frequent consumption of junk food can contribute to mental illness symptoms, even with minimal effects.

Abstract

The infectious disease from  corona virus, or COVID-19, has quickly spread world-wide since 2019. Therapies for managing COVID-19 have yet to be confirmed as medication for the severe sickness that the disease may cause. This study uses PRISMA guidelines as a method for conducting a systematic literature review. We selected relevant studies published from January 2020 to March 2020 by searching Pubmed, Science Direct and Google Scholar. The research focuses on the treatment of and therapies for COVID-19 only to the patients in the hospital. The inclution 8 articles  were screened by using a PRISMA flow after reading 28 abstract  and 28 from whole articles. Sixty-seven articles were collected from several online journal databases, and various therapies were found that are effective in the treatment and management of COVID-19. Clinical improvements and Recovery rate have been achieved in a short time compared to standard care only. The findings of the study show that several therapies are effective in managing the severe illness that can be caused by COVID-19. Nevertheless, research into effective therapies for COVID-19 must be continued to find the best therapy and treatment.

Abstract

Social health insurance from government program are expected to be able to reduce inequalities access to health services in the middle of rising of health care cost, while private health insurance is still limited for up and middle class population. This study aimed to analyze the equity level of health insurance ownership including social and private health insurance in Indonesia. This study examined the condition of Indonesia in the middle of entering National Health Insurance (NHI) era. This study used data of Indonesian Socio-Economic Survey 2012. Data were analyzed by using econometric approach through multinomial logit analysis. The results showed that the concentration index of social health insurance ownership was 0.615, which is smaller than private health insurance ownership (0.972). It means that Indonesia social health insurace ownership will be able to increase equity access to the health services especially for poor people (pro poor). Social health insurance ownership increases the use of the health services by people.

Abstract

Background: Risk mapping for the various types of non-communicable diseases (NCDs), which lead to the cause of death due to the impact of the urban flood, is rarely conducted. The study aimed to conduct a Systematic Literature Review (SLR) and meta-analysis of the risk of various NCDs after the community experienced the urban flood disaster. Methods: The data was taken from PubMed, ScienceDirect, Taylor and Francis, and ProQuest from June 2010 to June 2020 by searching for the words of flood and NCDs that only occurred in urban areas. This study refers to the guideline that is widely used to conduct meta-analysis research, namely PRISMA. The procedure was started by searching, screening, and validating the original articles through meta-analysis using R Studio. Result: Twenty-six original articles were selected for SLR, and only two articles were not eligible for meta-analysis. Mental illness (0.53, 95% CI, 0.33-0.68), heart problems (0.39, 95% CI -0.11-0.73), and respiratory problems (0.53, 95% CI, 0.02-0.82) are the NCDs categories occurring after the community, predominantly female and the community aged >40 years, was exposed to the urban flood. This risk magnitude finding complements the previous studies that only did a literature review. The heterogeneous result (tau^2 p-value>0.05) and bias effects of publication (Egger test p-value>0.05) indicated that immediate assistance for victims is essential to prevent the onset of NCDs. Conclusion: Without early prevention and urban planning, the urban flood disaster can contribute to mental illness, heart problems, and respiratory problems.

Abstract

Indonesia faces the combined challenges of a high prevalence of active smokers and an increased financial burden of smoking-related Non-Communicable Diseases (NCDs) on National Health Insurance (NHI). This study aims to assess the importance of increasing cigarette prices for NHI funding. It was a cross-sectional study conducted through a phone survey. The survey was administered to 1,000 respondents aged 18 and over from various telecommunications operators in Indonesia. We performed the multivariate analysis and the framework of the importance of increasing cigarette prices for funding NHI. The result shows that the majority of the public supports the increase of cigarette price for funding NHI except for active smokers at Odds Ratio=0.529. The importance of increasing cigarette prices for funding NHI is supported by society, able to reduce the prevalence of active smokers, and it raises the government’s tax income. With the current strong public support, cigarette prices should be increased which will then be tagged to fund the NHI consistently. This is also seen as a means to compensate the high NCD-related claims of the NHI which are caused by smoking behavior.JEL Classification  H51; I13; I18

Abstract

Target universal kepesertaan Jaminan Kesehatan Nasional (JKN) belum terpenuhi sedangkan kader posyandu belum banyak dilibatkan untuk membantu proses edukasi kepada masyarakat. Penelitian ini bertujuan untuk menganalisis kesiapan kader posyandu dalam melakukan kegiatan edukasi seputar program JKN kepada masyarakat. Jenis penelitian ini adalah kualitatif dengan pendekatan studi kasus. Informan penelitian yaitu kader posyandu. Analisis data dilakukan dengan mengulas hasil wawancara mendalam antar informan kader posyandu. Hasil penelitian menunjukkan bahwa edukasi terkait program JKN kepada masyarakat terbatas pada obrolan, di sela-sela kegiatan posyandu, diantara rapat RT dan pemanfaatan grup telekomunikasi. Dukungan yang diberikan dari Puskesmas masih terbatas dan kurangnya pelatihan informasi seputar program JKN kepada kader Posyandu. Penelitian ini menyimpulkan bahwa Belum ada kegiatan terencana, formal dan rutin yang diselenggarakan oleh kader untuk melakukan edukasi program JKN kepada masyarakat. Puskesmas bekerjasama dengan BPJS Kesehatan sebaiknya memberikan pelatihan rutin kepada kader posyandu mengenai informasi seputar program JKN agar edukasi yang disampaikan oleh kader kepada masyarakat semakin komprehensif.

Abstract

Jakarta sebagai salah satu pusat wilayah urban di Indonesia memiliki jumlah konsumen perokok elektronik terbanyak. Kelompok usia terbesar ada pada kelompok usia kerja. Tujuan dari penelitian untuk menganalisis penurunan produktivitas pada pengguna rokok elektronik kelompok usia produktif yang telah bekerja di wilayah urban (Jakarta). Jenis penelitian ini merupakan penelitian mix methods (kuantitatif dan kualitatif) yang dilakukan secara cross-sectional. Penelitian ini mendapatkan 44 sampel pengguna rokok elektronik. Karakteristik sampel penelitian ini sebagian besar bertempat tinggal di wilayah Jakarta Selatan, dengan sebagian besar jenis kelamin informan adalah laki-laki dengan beberapa responden perempuan dan pendidikan terakhir pada tingkat lulusan SMA. Hasil penelitian kualitatif menunjukkan bahwa perilaku merokok elektronik memiliki dampak negatif berupa keluhan terganggunya pekerjaan, peningkatan absensi kerja karena sakit, dan dampak yang disadari berupa penurunan kesehatan. Hasil penelitian kuantitatif menunjukkan bahwa perokok elektronik merasakan terganggunya pekerjaan karena sakit sebesar 56%, produk yang dihasilkan selama bekerja hanya sebesar 45%, dan merasakan terkena sariawan sebesar 31,8%. Penelitian ini menyimpulkan bahwa penurunan produktivitas kerja berpotensi terjadi pada pengguna rokok elektronik di daerah urban.

Abstract

Background: The COVID-19 pandemic in Indonesia has significantly impacted the health care system and insurance schemes. Social distancing policies during the pandemic have led to changes in utilization in health facilities, especially those related to catastrophic diseases that cost a lot of money. This study aims to analyse the impact of the Covid-19 pandemic on the utilization and financing of catastrophic diseases in the National Health Insurance (NHI) program. Methods: The research design is quantitatively and qualitatively. The quantitative study analysed secondary data obtained from the Social Security Administrator for Health and the nat. This study also has qualitative data from in-depth interviews with government and private hospital informants, health economists, and social health insurance experts. Results: COVID-19 pandemic has impacted decreasing the utilization of National Health Insurance participants to health facilities and significantly affecting the financing of catastrophic diseases. Before pandemic, the disease with the highest claim fee was ischemic heart disease, and after the pandemic, the disease with the highest claim fee was chronic kidney disease. There is also a change in the ranking diseases pattern before and during the covid-19 pandemic. Conclusion: In the future, National Health Insurance financing needs to expand the scope of the benefits of preventive and promotive services, especially the prevention of catastrophic diseases, to reduce the burden of NHI claims. Health facilities provided by National Health Insurance providers need to prepare for better management of health services after the pandemic.
