Abstract

Wabah Virus Zika (ZIKV) merebak pada tahun 2015 hingga 2016 terutama di benua Amerika dan mendapat perhatian dunia. ZIKV merupakan penyakit yang ditularkan oleh nyamuk Aedes, umumnya self limiting dan jarang fatal pada dewasa. Perhatian lebih kepada ibu hamil karena potensi janin mikrosefalus. Centers for Disease Control and Prevention (CDC) telah mengeluarkan beberapa rekomendasi diagnosis dan manajemen ZIKV baik pada ibu hamil maupun pada bayi baru lahir. Pencegahan dan diagnosis dini ZIKV menjadi kunci utama penanganan.

Abstract

Herpes zoster lebih sering terjadi pada kelompok usia di atas 50 tahun. Neuralgia pascaherpetika menjadi salah satu komplikasi tersering dan dapat menetap selama bertahun-tahun. Pengobatan neuralgia pascaherpetika saat ini belum adekuat. Pemberian vaksin herpes zoster menjadi strategi efektif untuk mengurangi insidens herpes zoster dan neuralgia pascaherpetika. Di Indonesia, vaksin herpes zoster telah direkomendasikan untuk seluruh individu berusia lebih dari 50 tahun.

Abstract

Background: To determine the incidence of hepatitis A infections and the clinical profiles of adult patients admitted to public hospitals in Jakarta, Indonesia. Methods: This was a cross-sectional study that utilised consecutive secondary data from internal medicine wards of seven public hospitals in Jakarta between 2011 and 2013. Eligibility criteria included patients over the age of 18 years and an ICD-10 diagnosis code of B15, acute hepatitis A. Case proportion was reported per 1000 people by dividing incidence per year to total in-ward patients. Clinical profiles were reported descriptively. Laboratory results were compared and categorised into groups of patients aged below and above 25 years old. Results: Data revealed that hospitalisations of patients with hepatitis A had decreased from 2011 to 2013. 289 patients were studied, the majority were young adults (18-25 years old) and their common chief complaints were nausea (36%), fever (24%), and jaundice (21%). Higher bilirubin levels were seen in older patients. There were 13 patients coinfected with hepatitis B, one patient coinfected with hepatitis C, and one patient coinfected with HIV. Conclusions: The proportion of hepatitis A infection amongst adults admitted to public hospitals in Jakarta was low and had decreased during the study period. Most of the patients reported classical clinical manifestations. This study found that the targeted age group may benefit from receiving routine hepatitis A vaccinations.

Abstract

Perlemakan Hati Non Alkoholik (Non Alcoholic Fatty Liver Disease/NAFLD) merupakan salah satu penyebab utama dari penyakit hati kronis di negara berkembang. NAFLD dapat berkembang menjadi menjadi penyakit hati yang lebih berat seperti Non Alcoholic Steatohepatis (NASH), sirosis hepatis, dan karsinoma hati. Tatalaksana NAFLD terus berkembang, dan saat ini klinisi dihadapkan dengan berbagai pilihan alternatif baik dari segi nonfarmakoterapi maupun farmakoterapi. Tulisan ini akan membahas berbagai pilihan terapi NAFLD terkini tersebut berdasarkan bukti terbaru.

Abstract

Severe malaria is a serious infectious disease that required immediate diagnosis and prompt treatment. Severe malaria may result several organ failures which is similar with other infectious diseases like SARS-CoV-2, sepsis, dengue, and HIV infection. Most of those infection have similar symptoms such as fever, myalgia, and headache, therefore physician should have high suspicion regarding co-infection with other infectious agents. The overlapping symptoms of co-infections may become a challenge for physician in diagnosing and delivering prompt therapy. Failure to deliver prompt treatment may lead to fatal outcome such as in severe malaria case. Stigmatization of all patients with fever and cough for COVID-19, may lead to misdiagnosis, moreover in some facilities which SARS-COV-2 PCR testing may not readily available to rule out infection. Therefore, it is importance to have broad differential diagnosis of fever during COVID-19 pandemic era, including the possibility of malaria infection in immunocompromised patients This can be achieved by comprehensive history taking and physical examination in evaluating patients with fever. We report antiretroviral naïve HIV infected patient with severe malaria indicated by several multi organ damage that successfully treated.
