Abstract

Obesity is a co-morbidity of cardiovascular disease, diabetes, and other metabolic diseases, which the prevalence increases every year. The use of herbal medicine is an alternative choice to overcome obesity. Clinical observation of decoction of herbal medicine (Dutch teak, kemuning, rhei radix and tempuyung) has been conducted for obesity therapy which has been proven to be effective in reducing body weight and body mass index (BMI). Innovation in steeping form is considered more practical than stew. Randomized open label pararel design research was carried out to assess the equal efficacy of the herbs in stew and steeping form. The number of subjects in each group were 30 subjects, they were patients of of Rumah Riset Jamu (RRJ) Hortus Medicus during June-October 2016 who met the inclusion criteria. BW and BMI are measured on Day-0, 28 and 56, while body height is measured once at Day-0. Both stew and steeping groups drank herbs twice a day for 56 days. At the end of the study, it was found that the mean of declining BW of the decoction group was 67.35 kg to 66.37 kg, while the steeping group 67.41 kg to 66.45 kg. BMI of stew group was decreased 26.85 kg / m2 to 26.46 kg / m2, while in steeping group 26.67 kg / m2 to 26.29 kg / m2. Statistic test result to compare BW and BMI of both two groups before and after treatment showed no significant differences in the two groups (p> 0.05). The ability of herbal medicines in steeping or stew forms can reduce the BW and BMI equally.

Abstract

Research on the safety of Jamu (herbal medicine) formula is important to guarantee its use in community. The Jamu Saintification development program has been resulted a cholesterol-lowering herb containing of 7 plants. This study aimed to determine the safety of the cholesterol-lowering herb. This study is a phase one of clinical trial with a pre-post group design, a single arm, involving 50 subjects with mild hypercholesterolemia without any complications and comorbidities. The intervention was conducted for 28 days at the Hortus Medicus Herbal Medicine Research House (RRJ) in 2014. Outcome parameters included clinical signs and symptoms, liver function laboratory tests (SGOT, SGPT), renal function (ureum, creatinine) and routine blood counts (hemoglobin, leukocyte, platelets, hematocrit). Subjects were requested to boil the herb everyday and drank it twice a day. The data were analyzed descriptively and statistically using SPSS. The results showed increasing in bowel frequencies of 92% subjects and abdominal pain in bowel movements of 34% subjects. These effects were still in a normal range and can be tolerated by the subjects. The paired t test analysis showed no differences in average laboratory parameters on day (D)-14 and D-28 compared to the baseline (p > 0.05). The cholesterol-lowering herbal medicine does not change the function of the liver, kidneys and blood, with mild side effects of increasing the frequencies of bowel movements and stomachaches.

Abstract

Hypertension is a risk factor for cardiovascular disease. Hypertension is one of the main causes of high mortality and morbidity in Indonesia. Utilization of medicinal herbs is an alternative for the treatment of hypertension. The study aimed to determine efficacy of the herbs combination (jamu) compared to hydrochlorothiazide (HCT) and evaluated the safety for hypertention stage I. This research method using an open-label randomized clinical trials with intervention for 56 days. Measurement of day-56 (D-56) showed that herbs decreased sistolic blood pressure to130.15 +17.59mmHg and diastolic at 83.82+9.53 mmHg. There was a significant decrease p = 0.000 (p 0.05), with p mean systolic blood pressure (p = 0.370) and mean diastolic blood pressure (p = 0.412)at D-56. Liver function parameters (SGOT, SGPT) and renal function (urea, creatinine) is still in normal range at the end of intervention. This study showed that herbs combination was an equally effective compared to HCT and safe for hypertension stage I.

Abstract

Background: Hypercholesterolemia is a risk factor for cardiovascular disease that can cause death and disability, so it must be managed properly. Garlic flour (GP) is one of the dosage forms of garlic that has the potential to reduce cholesterol. This study is a systematic review and meta-analysis that aims to determine the effectiveness of GP in reducing total cholesterol (TC) and low density lipoprotein (LDL).Subjects and Method: Search articles through the Pubmed database, Google scholar, Proquest, Springer link, and Science direct with the appropriate keywords. Population= patients with hypercholesterolemia. Intervention= GP. Comparison= placebo. Outcome= TC and LDL. Inclusion criteria included complete articles in English or Indonesian published from 2010-2022, Randomized Controlled Trial (RCT) design, subjects >18 years old with hypercholesterolemia, the study had a complete fat profile outcome, duration of intervention 2 weeks-12 months. The flow of article selection is based on the PRISMA flow diagram. Meta-analysis using RevMan 5.3 with effect size standardized mean difference (SMD) and Random Effect Model (REM) analysis model. Results: A total of 10 articles were eligible for a meta-analysis. The GP group had TC levels 1.49 units lower than the placebo group (SMD = -1.49; 95% CI = -1.95 to -0.55; p = 0.005) and LDL levels 0.68 units lower than the placebo group (SMD= -0.68; CI95). %= -1.08 to -0.27; p=0.001). Subgroup analysis based on the length of the intervention showed that administration of GP <12 weeks only significantly reduced TC while administration > 12 weeks significantly reduced TC and LDL.Conclusion: GP is effective in lowering TC and LDL especially at 12 weeks or more. Keywords: garlic powder, hypercholesterolemia, RCTCorrespondence: Zuraida Zulkarnain.
