Abstract

Objective: To report the congenital abnormalities of müllerian duct and review its literature. Methods: Case report. Case: A sixteen years old unmarried woman came to our policlinic with chief complain of cyclical abdominal pain (VAS 6). She had regular menstruation. In other hospital, she had diagnosed as suffering from ovarian tumor. Our examination showed that the uterus was unicornuate with rudimentary horn. We did explorative laparotomy. We took out the left cornu and left salpingoophorectomy due to hematosalphing and endometrial cyst. The Visual analogue score after surgery was reduced until VAS 2. Conclusion: Congenital abnormalities of the müllerian ducts are relatively common. Comprehensive examination can diagnose this abnormality earlier, thus intervention can be given as soon as possible. Abstract

Objective: To know the diagnostic value of a scoring system taken before surgery and frozen section in young-aged patients with suspected malignancy. Using that result, we can also investigate whether frozen section gives additional value to clinical scoring system. Method: This study is a diagnostic test. This study was carried out by accessing RSCM’s medical record from 2006-2011. From 437 patients suspected of ovarian malignancy, we included 157 patients due to age. Result: Diagnostic value of GP score are 77%, 49%, 61%, 68%, 63%, while RMI are 69%, 49%, 58%, 45%, 59%, (sensitivity, specificity, positive predictive value, negative predictive value, and accuracy respectively). Diagnostic value of frozen section in patients with suspected malignancy using GP score >4 are 81.7%, 87.2%, 90.7%, 75.6%, 83%, while in patients with RMI 200 are 81%, 87%, 89%, 77%, 83% (sensitivity, spesificity, positive predictive value, negative predictive value, and accuracy respectively). Conclusion: Gatot Purwoto score and RMI have good diagnostic value in proving malignancy in young age. Its predictive value will be increased by frozen section. Abstract

Objective: Complete heart block is an extremely rare and serious complication in pregnancy. Pregnancy outcome in patients with atrioventricular conduction block are unknown, with only a limited number of case reports published. This paper is aimed to report our case and review the available background literature. Method: Case report. Case: A twenty‐nine years old primigravida in labor presented at 38 weeks of gestation with referral from the primary health center due to bradycardia and her previous history of cardiac problem. Patient has been diagnosed with total atrioventricular block since 2 years ago. Electrocardiography assessment showed the presence of complete heart block. She was then planned for an emergency Caesarean section and later a temporary transvenous pacemaker was implanted. Conclusion: Management of complete atrioventricular block in pregnancy requires a good team consisting of obstetrician, anesthesiologist and cardiologist.
