Abstract

In connection with the transmission of the HIV virus, housewives may have experienced internal stigma, which can have an effect on HIV prevention and the treatment of AIDS. Twenty-three HIV-infected housewives were interviewed in a qualitative study conducted in North Jakarta. The data were analyzed using a phenomenological approach. Seven themes emerged to explore internal stigma housewives and to compose the essence of the internal stigma. The essence of an HIV-infected housewife who has internalized the stigma can be illustrated as an Indonesian proverb which is “Hidup Segan Mati Tak Mau” or in English can be interpreted as ‘having lost the will to live, but not wanting to die’. This could explain that AIDS forces them to live in anxiety in all aspects of life but as housewives, they must keep thinking about continuing life especially with regard to the future of their beloved children.

Abstract

ABSTRACTIntroduction: Tuberculosis is a major global health problem. It is estimated that almost 9 million new cases detected in 2011 and 1.4 million died because of TB. Early diagnosis and effective treatment are the key elements of the TB control program. Delayed diagnosis associated with a longer duration of infectiousness, increase the severity of the disease, more severe complication and even death.Objectives: To determine the factors associated with delayed diagnosis among tuberculosis patients in Kebumen District.Methods : A cross sectional study among newly diagnosed TB patients in April to June 2013 in Kebumen was conducted. The data was collected using a questionnaire interviewing officers of government health care facilities which have implemented DOTS programs already. Data was analyzed by a logistic regression test with confidence interval (CI) of 95%.Results: Eighty five pulmonary TB patients were studied consisting of 65 TB smear positive and 20 smear negative/roentgen positive patients. The median duration of delayed patients was 3.9 weeks; delayed health service was 2.6 weeks and delayed diagnosis was 7.6 weeks. Multivariate analysis showed that factors associated with delayed diagnosis were the type of health services which first visited (p value = 0.002 and OR = 6.87), level of knowledge about TB (p value = 0.002 and OR = 6.41), educational level (p value = 0.024 and OR = 5.68) and the number of visits to health service (p value = 0.021 and OR = 3.87).Conclusion : The median duration of delayed diagnosis was 7.6 weeks. Delayed diagnosis among TB patients in Kebumen Distict was associated with type of health services which is non-DOTS, low level of knowledge about TB,  low levels of education and the number of visits to health services ≥3 times.Keywords: delayed diagnosis, TB patients, Kebumen District

governmental organizations in the implementation of quality DOTS known as Public Private Mix (PPM). Ende has executed the implementation of PPM DOTS approach involving government hospitals, private hospitals and private clinics since 2010, but the program has never been evaluated to determine the problem and found a solution to improve the performance of the TB control program in Ende. Objectives: To evaluate the implementation of the Public Private Mix (PPM) for TB control in Ende Methods: This study used a qualitative design, with descriptive case study approach, to describe the implementation of PPM for TB control in Ende, a district of East Nusa Tenggara Province. Result and Discussion: PPM implementation of TB control in Ende not run optimally, this showed by the low performance of the TB control program that is low TB case detection rate (CDR) <70%, although there was an increase in TB case detection (CDR) of 12% , treatment success rate <85%, the conversion rate <80% and the high dropout rate of TB patients> 10% in the last three years. Factors - factors that inhibited the implementation of PPM for TB control are shortage of human resources, insufficient budget, lack of logistics and facilities infrastructures of TB DOTS unit and dependence on donor resources, the absence of operational guidelines governing cooperation mechanisms, the lack of commitment of the government and partners in the implementation of PPM control TB, lack of communication and coordination between PPM network, caseholding of TB patients. Conclusion: Implementation of PPM TB Control has not been optimal as it has not improve the performance of the TB control program in the district of Ende yet. Latar belakang: Salah satu komponen strategi stop TB adalah melibatkan seluruh penyedia layanan pemerintah, swasta, lembaga swadaya masyarakat dalam pelaksanaan DOTS yang berkualitas yang dikenal dengan Public Private Mix (PPM). Kabupaten Ende sejak tahun 2010 telah melaksanakan pendekatan PPM dalam pelaksanaan DOTS yang melibatkan rumah sakit pemerintah, rumah sakit swasta dan balai pengobatan swasta, namun belum pernah dilakukan evaluasi untuk mengetahui permasalahan dan ditemukan solusi untuk memperbaiki kinerja program pengendalian TB di Kabupaten Ende. Tujuan Penelitian: Untuk mengevaluasi implementasi Public Private Mix (PPM) pengendalian TB di Kabupaten Ende. Metode Penelitian: Penelitian ini menggunakan rancangan kualitatif, dengan pendekatan studi kasus deskriptif, untuk menggambarkan pelaksanaan PPM pengendalian TB di Kabupaten Ende Provinsi Nusa Tenggara Timur. Hasil dan Pembahasan: Implementasi PPM pengendalian TB di Kabupaten Ende belum berjalan optimal, ini terlihat dari masih rendahnya kinerja program pengendalian TB yaitu rendahnya angka penemuan kasus TB (CDR) < 70%, meskipun ada peningkatan penemuan kasus TB (CDR) sebesar 12 %, angka keberhasilan pengobatan < 85%, angka konversi < 80% dan tingginya angka drop out pasien TB > 10% dalam kurun waktu tiga tahun terakhir. Faktor – faktor yang menghambat implementasi PPM pengendalian TB adalah keterbatasan sumber daya manusia, anggaran, logistik TB dan sarana prasarana unit DOTS serta ketergantungan sumber daya terhadap pihak donor, tidak adanya pedoman operasional yang mengatur mekanisme kerjasama, kurangnya komitmen pemerintah maupun mitra dalam implementasi PPM pengendalian TB, kurangnya komunikasi dan koordinasi antara jejaring PPM dalam menjaga keteraturan dan keberlangsungan pengobatan penderita TB.Kesimpulan: Implementasi PPM Pengendalian TB belum berjalan optimal karena belum dapat meningkatkan kinerja Program Pengendalian TB di Kabupaten Ende

Abstract

Latar Belakang dan Tujuan: Stroke bukan hanya menyebabkan beban dalam dunia kesehatan akan tetapi juga membebani baik dari segi ekonomi suatu negara, dan juga secara psikologis, terutama pada keluarga dengan pasien yang mengalami kecacatan total. Biaya langsung maupun tidak langsung dari perawatan stroke diperkirakan mencapai lebih dari 65 miliar US dollar, sehingga stroke dapat digolongkan sebagai penyakit termahal setara dengan penyakit kronis diabetes dan gangguan depresi. Adanya variasi dalam pelayanan kesehatan diperkirakan membawa dampak terhadap biaya yang tidak tetap dan bahkan berlebihan. Panduan tertulis berupa Clinical Pathway (CP) diharapkan mampu mengurangi variasi biaya ini, dengan tetap mempertahankan kualitas yang baik. Metoda Penelitian: Penelitian ini merupakan penelitian observasional inferensial, dengan metoda Retrospektif Kohort. Data yang diambil adalah data sekunder di RS Bethesda Yogyakarta, berupa database komputer. Sampel yang diambil berjumlah 130 orang, baik pada kelompok kontrol (Perawatan tanpa CP), maupun pada kelompok observasi (Perawatan dengan CP). Hasil: Terdapat perbedaan signifikan terhadap biaya perawatan stroke iskemik akut setelah penerapan CP (p=0,004), dimana rerata biaya perawatan pada kelompok dengan CP sebesar Rp 8.212.656,02 dan pada kelompok tanpa CP sebesar Rp 10.659.617,72 (setelah penyesuaian dengan tingkat inflasi dari BPSI sebesar 19,08%) dengan beda rerata sebesar Rp 2.446.961,70. Kesimpulan: Clinical Pathway mampu memberikan penurunan biaya perawatan terhadap perawatan stroke iskemik akut di RS. Bethesda Yogyakarta.

Abstract

More than two-thirds of deaths in children are related to improper feeding practices, especially in adolescent mothers, with very low levels of complementary feeding knowledge, less responsiveness, and few skills of proper feeding practices.  The area of Banguntapan III Public Health Center (PHC) has the highest prevalence of adolescent mothers in 2018-2019. The aim of this study was to evaluate the effect of WhatsApp-based complementary feeding education and counseling for adolescent mothers who have children aged 6-24 months at Banguntapan III PHC on complementary feeding knowledge and practice. A quasi-experimental with one group pre-post test was conducted (n=10 adolescent mothers) on February-May 2021 at the Banguntapan III PHC area. The education program was conducted by trained cadres for 8 days through the WhatsApp group, then followed by asynchronous counseling once through WhatsApp private chat. Change of outcome was tested using independent sample T-test and McNemar test. The range of adolescent mothers’ age in this study was 19-20 years, with education level of junior high school (50%), and do not work (80%). The results showed that WhatsApp-based education and counselling had a significant effect on increasing the adolescent mother's IYCF knowledge (Δ = 1.1 ± 2.28) with a p-value of 0.017 < a (0.05). Meanwhile, the effect of the intervention on the IYCF practice by adolescent mothers was not significant, with a p-value of 0.311 > a (0.05). In conclusion, WhatsApp-based education and counselling for adolescent mothers can increase their level of complementary feeding knowledge, but was not been able to improve their feeding practices for infants/children aged 6-24 months.

Abstract

Background: Tuberculosis (TB) disease is a major health problem either in developed or developing countries. According to WHO TB happens to 25,205 people worldwide everyday,  ending in mortality to 4,657 people. Indonesia is in the third rank of TB cases in the world after India and China. TB in Indonesia happens to 1,464 people everyday, ending in mortality to 241 people. The identification of suspect and positive acid fast bacillus is a performance indicator of lung TB program. At District of Bengkulu 2007 lung TB patients’ identification was still below the national target, i.e. 38.193. The screening and identification of patients is generally done by nurses at polyclinics and laboratory staff. Nurses are health professionals at the front liners of health services with quite large composition (40%).Objective: To identify factors of the health center in relation to the coverage of positive acid fast bacillus lung TB patients at District of Bengkulu Utara.Method: This was an observational study with a cross sectional design. Location of the study was microscopic referral health centers, independently managed health centers, and satellite health centers at District of Bengkulu Utara. Samples consisted of staff of polyclinic and laboratories. Data were obtained through questionnaire and observation and presented using frequency distribution tables and multilevel regression and linear regression methods.Result: The result of multivariate analysis showed that variables related to the coverage of lung TB patients were skills of staff with b=1.3 (p=0.000) and facilities with b=1.5 (p=0.000).Conclusion: Skills of staff, training of staff, and availability of facilities were related to the coverage of positive acid fast bacillus lung TB patients at District of Bengkulu Utara; therefore it was necessary to improve skills of staff through on the job training and improve facilities for the identification of lung TB patients.Keywords: lung TB, nurses, laboratory staff, TB patient identification

Abstract

Community perception of measles immunization in SlemanPurposeThis study conducted to explore the community’s perception of measles immunization in Sleman.MethodsA phenomenological study was conducted involving interviews of the head of neighborhood’s ladies who were highly respected by their citizens, mothers with toddlers but refused immunizations, immunized toddler mothers, health workers who did not immunize their children based on information from FGDs of mothers who refused immunization, a toddler father who does not allow his child to be immunized, community leaders and religious leaders, in the village of Sukoharjo whose area has been infected with measles outbreaks. ResultsThe study found that first, people perceived that immunization has no benefit and even causes side effects for children’s under-five health. Second, the community perceives that immunization of measles is not mandatory because of the lack of the government's role in enforcing regulations related to measles immunization. Third, the perception of people against measles immunization is influenced by the behavior of health workers and religious leaders. ConclusionsThe side effects of measles immunization, the influence of health workers' behavior and religious figures related to non-immunization against measles, and the lack of government's role in enforcing regulations related to the appeal of measles immunization are the factors that influence community's perception and a decision of not doing measles immunization. Government appeals related to immunization need to be improved, and in addition, education related to measles hazard should be done to the community, religious leaders and health workers to increase awareness and awareness of the dangers of measles.

Abstract

Determinant factors affecting the use of insecticide-treated nets in Bengkulu: analysis from basic health survey 2013PurposeThis study aimed to identify determinant factors that influence the use of insecticide-treated nets and to evaluate the effectivity of the mosquito nets usage, in pregnant women and young children in Bengkulu, based on  basic health survey 2013.MethodThis study used a cross-sectional design. Study population were all members of households in Bengkulu that used insecticide-treated nets, based on basic health survey 2013.ResultsThere were no significant correlations between employment status, education, vulnerable children, use of community health center, housing, and use of wire gauze with insecticide-treated nets. Multivariate logistic regression tests showed that there were some socio-demographic variables, behavior, and environment that affected mosquito net usage.ConclusionSocial determinants of demographic factors, behavioral determinant factors, environmental determinant factors, and presence of children affected the use of insecticide-treated nets, but the use of mosquito nets was not effective against vulnerable groups (pregnant women).

Abstract

Role of the head of public health centers from public health graduates and others on improving managerial skills in East OkuPurposeThis study aimed to understand how public health and non-public health backgrounds affect management skill of primary health care heads, based on ten managerial roles. MethodsThis study was a descriptive study with case-series method, using qualitative methods supported by quantitative data. Eight people were recruited as respondents. They consisted of 4 people that were primary healthcare heads with public health background and 4 people that were primary healthcare heads with non-public health background. Respondents were recruited by purposive sampling. Data collection was performed using in-depth interviews and questionnaire. ResultsPrimary healthcare head with public health background owned more work experience than those with non-public health background. Period of employment in primary health care had an influence on managerial capabilities. There were no significant differences of period of employment in primary health care head positions among the longest with the least to the managerial capabilities. Primary health care heads who had been trained with PIM IV training and other structural positions were known to have better management capabilities, especially compared to the leadership role of those who had not been trained.ConclusionThere was no significant difference in managerial capacity between primary health care heads with public health and those with non-public health background.

Abstract

Analysis of 5-year survival of patients treated for breast cancer at Sardjito Hospital in Yogyakarta province, IndonesiaPurposeThis study aimed to assess the length of life of breast cancer patients as well as factors related to the prognosis of survival of patients.MethodsThis research was a retrospective study. Samples in this study were patients with breast cancer who were first diagnosed with breast cancer from January 1, 2009 until December 31, 2009 at the Dr. Sardjito Hospital, Yogyakarta. The Kaplan Meier method was used for the data analysis, and final factors were reviewed and those that showed significant association were entered in a Cox regression model. Calculations were performed in Stata 12.0.ResultsResults showed the 5 year survival rate was 51.07%. In bivariate analysis, there was a correlation between the age of diagnosis, education, clinical stage, tumor size and tumor location on 5-year survival in patients with breast cancer. For multivariate analysis, age of diagnosis had the most powerful correlation (HR = 3.73; 95% CI = 1.0 to 13.6) on survival (p = 0.046).ConclusionSurvival rates of women with breast cancer aged less than 50 years were lower. Young women with breast cancer tended to have more aggressive breast cancer growth and recurrence risks were greater. Efforts are needed for early detection of breast cancer to improve survival in breast cancer patients.
