    In my opinion, we should follow below approach. 1. Identify and analyse the problem thoroughly. Prepare a report including some data showing the problem. It should clearly include the problem statement and some reasoning around it like why it occurred. This will allow everyone to understand clearly about the RCA of the problem also the data will help to get in depth of it. 2. Then there should be some possible solutions to the problem focused on the minimal impact to the business to discuss. If there are multiple of such solutions then they should be presented individually with some data depicting what they bring in and how they effectively solve the problem. Also solutions should focus on how they can ignore the problem in future.
